目的探讨妊娠高脂血症患者血浆高密度脂蛋白(HDL)亚类的分布特征及相关脂酶的变化。方法采集2015年8-10月在中国人民解放军第202医院就诊的50例妊娠高脂血症患者和50例正常孕妇的血浆,采用Lipoprint脂蛋白亚类分析系统检测血浆HDL亚类组成和含量;Elisa法检测血浆肝脂酶(HPL)、脂蛋白脂肪酶(LPL)含量。结果妊娠高脂血症患者血浆大颗粒HDL浓度及百分比[(0.74±0.23)mmol/L、(39.43±6.71)%]明显低于正常孕妇[(0.96±0.28)mmol/L、(45.95±6.95)%],差异有统计学意义(P<0.05);中颗粒HDL百分比明显高于正常孕妇,差异有统计学意义(P<0.05);妊娠高脂血症患者血浆HPL、LPL浓度明显低于正常孕妇,差异有统计学意义(P<0.05)。结论妊娠高脂血症患者血浆HDL亚类分布特征的改变,即大颗粒HDL减少,较HDL浓度改变更具有临床指导意义。妊娠高脂血症患者血浆HPL、LPL可能作为临床检测妊娠高脂血症病情变化的新指标。
Objective To investigate the distribution of plasma high density lipoprotein (HDL) subtypes and the changes of related lipase in patients with pregnancy induced hyperlipidemia. Methods The plasma of 50 pregnant women with hyperlipidemia and 50 normal pregnant women attending the 202 Hospital of People’s Liberation Army from August 2015 to October 2015 were collected. Lipoprint lipoprotein subclass analysis system was used to detect plasma HDL subclass composition and content. Elisa method was used to detect plasma hepatic lipase (HPL) and lipoprotein lipase (LPL). Results The plasma level of HDL in plasma of pregnant women with hyperlipidemia was significantly lower than that of normal pregnant women [(0.74 ± 0.23) mmol / L, (39.43 ± 6.71)% vs [(0.96 ± 0.28) mmol / L, )%], The difference was statistically significant (P <0.05). The percentage of HDL in middle granule was significantly higher than that in normal pregnant women (P <0.05). The plasma concentrations of HPL and LPL were significantly lower in patients with hyperlipidemia Normal pregnant women, the difference was statistically significant (P <0.05). Conclusion The change of plasma HDL subclass distribution in pregnant patients with hyperlipidemia, that is, the decrease of HDL in large particles, is more clinically instructive than the change of HDL concentration. Plasma hyperlipidemia in patients with plasma HPL, LPL may be used as clinical indicators of pregnancy hyperlipidemia changes in the new indicators.
