目的:了解精索静脉曲张对小儿睾丸的影响。方法:43例患儿中,B 超观察22例术前及28例术后患儿双侧睾丸体积差异,并分别于光镜和电镜下观察13例及9例患侧睾丸标本的病理改变。结果:术前患侧睾丸体积明显小于健侧(P<0.01)。术后睾丸体积有增大改变,但1年内仍小于健侧,3年后同健侧。光镜下10例睾丸标本可见曲细精管界膜增厚和末成熟的精细胞脱落。电镜下3例睾丸标本支持细胞和间质细胞有改变。结论:小儿精索静脉曲张影响睾丸的发育,治疗后睾丸体积能逐渐恢复,提示这种改变是可逆的,故确诊后及早治疗对保征睾丸的正常发育颇为重要。
Objective: To understand the influence of varicocele on children’s testes. Methods: Bilateral ultrasonography was used to detect the difference of testicular volume in 22 cases of preoperative and 28 cases of postoperative children. Pathological changes of 13 cases and 9 cases of testis were observed under light microscope and electron microscope respectively. Results: The preoperative ipsilateral testicular volume was significantly smaller than that of the contralateral side (P <0.01). Postoperative testicular volume increased change, but within 1 year is still less than the contralateral, 3 years after the same side. Light microscope, 10 cases of testicular specimens can be seen in seminiferous tubule thickening and terminal mature sperm cell shedding. Three cases of testis specimens under the electron microscope showed changes in the number of supporting cells and interstitial cells. Conclusion: Pediatric varicocele affects the development of testis and the volume of testes gradually recovered after treatment, suggesting that this change is reversible. Therefore, early diagnosis after diagnosis is quite important for the normal development of testicular preservation.
