目的:探讨替牙期上颌骨发育不足所致反采用前方牵引器矫治治疗的临床效果。方法:将笔者所在医院近年来收治的60例替牙期上颌骨发育不足所致反患者按照随机数字表法分为观察组与对照组,每组30例。观察组采用前方牵引前快速扩弓治疗,对照组在牵引治疗中不进行扩张,治疗前、后采用传统分析项目、McNamara测量法对患者投影进行测量分析。结果:两组患者治疗时间为4~12个月,平均治疗时间为(8.41±3.22)个月,两组患者上牙弓前移、上颌骨、前牙反均得到明显纠正,与治疗前比较差异有统计学意义(P<0.05)。观察组纠正程度明显大于对照组,两组比较差异有统计学意义(P<0.05)。观察组下颌骨后下旋转改善程度明显优于对照组,两组比较差异有统计学意义(P<0.05)。结论:前方牵引器矫治治疗替牙期上颌骨发育不足所致反效果显著,在治疗同时进行扩张有助于下颌骨及软组织侧貌更好改善,可在临床推广使用。
Objective: To investigate the clinical effect of orthodontic treatment of anterior maxilla caused by undeveloped maxilla during maxillary dentition. Methods: The author’s hospital in recent years admitted to 60 cases of maxillary hypoplasia due to period of anti-patients caused by random number table divided into observation group and control group, 30 cases in each group. The observation group was treated with rapid expansion before anterior traction while the control group was not expanded during traction treatment. The traditional analysis was performed before and after treatment, and the projection of the patient was measured by McNamara measurement. Results: The treatment time ranged from 4 to 12 months and the average duration of treatment was (8.41 ± 3.22) months. The maxillary and anterior teeth of the two groups were anteriorly corrected, while the anterior and posterior teeth of the two groups were significantly reversed. Compared with those before treatment The difference was statistically significant (P <0.05). The degree of correction in the observation group was significantly greater than that in the control group, with significant difference between the two groups (P <0.05). The degree of mandibular retrophael rotation in the observation group was significantly better than that in the control group, with significant difference between the two groups (P <0.05). Conclusion: The frontal traction appliance is effective in treating the periodontal maxillary hypoplasia. It is effective in the simultaneous improvement of mandibular and soft tissue profile, which can be widely used clinically.
