目的分析门诊退药原因,以减少和避免退药现象的发生,保证患者用药安全有效。方法抽查北京军区总医院门诊2010年3~10月的368份退药处方,并对其进行了科学分析,探讨应对策略。结果门诊退药原因主要有医方(医生缺乏责任心、跟患者缺少沟通、不合理用药)、患方(患者缺乏医学知识、患者用药依从性差、患者入院、病情变化)药方(包装有瑕疵或近效期出售、供应短缺)、其他(如药品不良反应、其它)等几方面原因引起。结论加强医生、药师的责任心,加强医患沟通,普及患者用药知识,规范退药程序,建立严格的退药制度,在用药安全、医院利益和患者利益间求得合理的平衡。
Objective To analyze the reason of withdrawal from clinic, to reduce and avoid the occurrence of withdrawal phenomenon and to ensure the safe and effective treatment of patients. Methods A total of 368 prescriptions for withdrawal from March to October 2010 in the General Hospital of Beijing Military Region were randomly selected and scientifically analyzed to explore coping strategies. Results The main reasons for withdrawal from clinic were doctors (lack of sense of responsibility, lack of communication with patients and irrational use of drugs), patients (lack of medical knowledge, poor medication compliance, admission of patients and changes in condition) Near term sale, supply shortage), other (such as adverse drug reactions, others) and other causes. Conclusion To strengthen the doctor’s and pharmacist’s responsibility, strengthen the communication between doctors and patients, popularize the medication knowledge of patients, standardize the withdrawal procedure, establish a strict withdrawal system, and find a reasonable balance between drug safety, hospital interests and patients’ interests.
