目的探讨肢体静脉畸形伴发关节功能障碍的相关危险因素。方法 2013年1月至2015年8月,收集肢体单纯静脉畸形患者临床和影像学资料,初诊时伴发关节功能障碍者作为功能障碍组,未出现功能障碍者为对照组,对两组患者的资料进行相关危险因素分析。结果共168位肢体静脉畸形患者入组,其中功能障碍组44例,临床表现为关节屈曲挛缩或伸直受限,以及关节外翻或内翻畸形。单因素分析显示,功能障碍与患者年龄、部位、病灶大小、累及组织、疼痛评分及持续时间有统计学相关性(P<0.05)。非条件多因素Logistic回归分析显示,年龄≤18岁、下肢病灶以及疼痛时间超过1年,是肢体静脉畸形伴发功能障碍的独立危险因素。结论对于肢体静脉畸形除了早期诊断,更要加强对疼痛的早期控制和有效缓解。
Objective To explore the related risk factors of limbal venous malformation associated with joint dysfunction. Methods From January 2013 to August 2015, clinical and imaging data of patients with simple venous malformations of the limbs were collected. At the time of first visit, those with dysfunctional joints were regarded as dysfunction group and those without functional impairment as control group. Data analysis of the relevant risk factors. Results A total of 168 patients with limb venous malformations were enrolled, including 44 patients with dysfunction group. The clinical manifestations were joint flexion contracture or limited straightness, and valgus or varus deformity. Univariate analysis showed that dysfunction was related to age, location, size of lesion, tissue involvement, pain score and duration (P <0.05). Unconditional multivariate Logistic regression analysis showed that age ≤18 years old, lower extremity lesions and pain lasting more than one year were independent risk factors of limb venous malformation associated with dysfunction. Conclusion In addition to early diagnosis of limb venous malformation, but also to strengthen the early control of pain and effective relief.
