[目的]结合临床实践经验,探讨尿中肾损伤标志物检测在儿科疾病诊断中的应用价值。[方法]采用对照研究,被检儿童均留取新鲜尿液(除晨尿)10ml,-20℃保存待检。试剂、标准品、质控品由上海太阳生物技术公司提供。离心后标本用ELISA法测定IgG、RBP、β2-MG、m-ALB,在酶标仪492nm处读OD值计算结果,并采用比色法测NAG、721分光光度计405nm处比色并计算结果。对比分析两组的检测结果,分析尿中肾损伤标志物检测在儿科疾病诊断中的应用价值。[结果]肾病组尿中标志物水平均显著高于正常对照组,差异有统计学意义(P﹤0.05),肾病组各肾损伤标志物测定水平较高,联合应用100%阳性,而正常对照组仅有5.00%为阳性(主要为IgG水平较高,考虑主要与其出现在非选择性蛋白尿有关),两组比较差异有统计学意义(χ2=21.93,P﹤0.05)。[结论]对于多种原因造成的肾脏损伤,可以检测尿中IgG、RBP、β2-MG、m-ALB、NAG水平,不仅比血中BUN、Cr等肾功能指标敏感,而且实验标本留取简便,有助于临床早期发现肾损伤,积极采取合理用药并判断预后。
[Objective] To explore the value of detecting urinary renal injury markers in the diagnosis of pediatric diseases based on clinical practice. [Methods] According to the control study, 10ml of fresh urine (except morning urine) were collected from children under test and stored at -20 ℃ for examination. [Results] The levels of urinary markers in nephropathy group were significantly higher than those in normal control group (P <0.05). The levels of markers of kidney injury in nephropathy group were higher than those in normal control group (100% positive) Group, only 5.00% was positive (mainly for the higher level of IgG, consider the main occurrence of non-selective proteinuria), the difference between the two groups was statistically significant (χ2 = 21.93, P <0.05). [Conclusion] It is not only sensitive to renal function indexes such as BUN and Cr in blood, but also can be used to test IgG, RBP, β2-MG, m-ALB and NAG in urine for a variety of reasons. , Helps to find early renal damage in clinical, and actively take reasonable medication and determine the prognosis.
