他莫昔芬常见副作用为潮红、月经不调、恶心、呕吐、骨癌及肿瘤部位疼痛或潮红，此外，早在1978年即有4例高剂量（每天120～180mg）应用他莫昔芬致眼性病变的报道，上述患者平均用药时间为17～27个月，用药量累计达108～230g，4例皆出现折射性视网膜浑浊，其中3例患者视力下降
