目的:探讨血清βn 2微球蛋白(βn 2-MG)、乳酸脱氢酶(LDH)、血清白蛋白(ALB)、C反应蛋白(CRP)水平检测对多发性骨髓瘤(MM)的临床应用价值。n 方法:采用完全随机设计,选取中山大学附属第三医院血液内科2018年4月至2021年3月收治的MM患者25例(男性12例,女性13例,中位年龄63岁),列为MM组;选取2021年1月肾内科收治的肾功能减退患者20例(男性11例,女性9例,中位年龄62岁),列为肾内科组,同期血液科收治的非MM血液病患者20例(男性12例,女性8例,中位年龄44岁),列为血液科组。对3组患者的βn 2-MG、LDH、ALB、CRP水平进行两两比较,并做统计学分析。n 结果:肾内科组患者的βn 2-MG为16.16(6.21,30.24)mg/L,MM组、血液科组患者分别为4.20(2.45,7.85)mg/L、2.58(2.32,4.32)mg/L,肾内科组与MM、血液科组相比较,差异均有统计学意义(均n P<0.001),MM组与血液科组的βn 2-MG相比较,差异无统计学意义(n P>0.05);而MM、肾内科及血液科组患者的血清LDH、ALB、CRP水平两两相比较,差异均无统计学意义(n P>0.05)。n 结论:血清βn 2-MG、LDH、ALB、CRP的检测对MM有重要的临床应用价值,但以上指标水平升高在肾功能减退、血液病中也可存在,单独检测较易出现诊断误差。n “,”Objective:To investigate the clinical value of detecting serum levels of β2 microglobulin (βn 2-MG), lactic dehydrogenase (LDH), serum albumin (ALB), and C-reactive protein (CRP) in multiple myeloma (MM).n Methods:The study design was a completely random design. A total of 25 MM patients who were 27-82 years old and were admitted to Department of Haematology, The Third Affiliated Hospital of Sun Yat-sen University from April 2018 to March 2021, including 12 males and 13 females, were selected as an MM group; 20 renal dysfunction patients who were admitted to Department of Nephrology of our hospital in January 2021, including 11 males and 9 females, were set as a nephrological department group, with a median age of 63; and 20 patients with non-MM hematologic diseases who were admitted to Department of Hematology at our hospital, including 12 males and 8 females, were selected as a hematological department group, with a median age of 44. The levels of βn 2-MG, LDH, ALB, and CRP were compared between these 3 groups, and were statistically analyzed.n Results:The βn 2-MG level in the nephrological department group[16.16 (6.21, 30.24) mg/L] was statistically different from those in the MM group [4.20(2.45, 7.85) mg/L] and the hematological department group[2.58 (2.32, 4.32) mg/L],bothn P<0.05. There were no statistical differences in the serum levels of LDH, ALB, and CRP between these 3 groups (all n P>0.05).n Conclusion:The detection of serum βn 2-MG, LDH, ALB, and CRP has important clinical application value in multiple myeloma, but the elevated levels of these indicators can also exist in patients with renal dysfunction and hematologic diseases, and the single detection is likely to cause diagnostic errors.n
