目的探讨实时超声评价正中神经活动度联合横截面积测量诊断腕管综合征(CTS)的意义。方法单侧非急性CTS患者43例,实时超声评价正中神经活动度及测量豌豆骨水平的横截面积(CSAP),与健侧手及对照组比较,术前与术后(6个月)比较。结果正中神经移动试验(MTUS)阳性率和CSAP测值在CTS患侧与健侧手及对照组比较、术前与术后比较均有统计学意义(均P<0.05)。联合MTUS阳性和CSAP≥10.0mm2诊断CTS,采用平行或系列试验法可以提高其诊断的灵敏度或特异度。结论 MTUS可作为CTS正中神经动力学异常的高特异度检出方法。
Objective To evaluate the value of real-time ultrasound in the diagnosis of carpal tunnel syndrome (CTS) with median nerve activity combined with cross-sectional area measurement. Methods Forty-three patients with unilateral CTS were examined by real-time ultrasound to evaluate the median nerve activity and to measure the cross-sectional area (CSAP) of pea bones. Compared with contralateral hand and control group, preoperative and postoperative (6 months) . Results The positive rate of positive neurotransmitter test (MTUS) and the value of CSAP in the CTS affected side compared with the contralateral hand and the control group were significantly different (all P <0.05). Joint MTUS positive and CSAP ≥ 10.0mm2 diagnosis of CTS, using parallel or series of tests can increase the sensitivity or specificity of its diagnosis. Conclusion MTUS can be used as a high-specificity detection of CTS median nerve dynamic abnormalities.
