目的调查重庆市不同人群中脂肪肝患病率和分析相关因素。方法采用横断面现况调查方式,调查重庆市农民、工厂工人、学校教师和机关干部中脂肪肝患病率,内容包括填写问卷,测定身高、体重、腰围、臀围、血压、腹部体检和B超检查等。结果共调查2457例,检出脂肪肝427例,脂肪肝患病率为17.38%,男性患病率为19.67%,女性患病率为13.73%,男性脂肪肝患病率显著高于女性(χ2=14.31,P<0.001)。脂肪肝患病率随着年龄增加而上升(χ2=60.97,P<0.0001)。不同职业群体中脂肪肝患病率教师23.58%、干部20.94%、工人19.21%和农民6.32%。农民组最低,与城市中教师、干部和工人比较有非常显著差异(χ2=70.16,P<0.0001)。经单因素和Logistic多元回归分析,以BMI、腹型肥胖、高血压病和糖尿病与脂肪肝密切相关,其中BMI和腹型肥胖相对风险度最高(OR分别为3.0984和3.0305)。结论重庆市脂肪肝患病率为17.38%,与上海市报道十分接近。农民脂肪肝患病率显著低于城市中教师、干部和工人。肥胖是脂肪肝最主要的危险因素。
Objective To investigate the prevalence and risk factors of fatty liver among different population in Chongqing. Methods The prevalence of fatty liver in Chongqing farmers, factory workers, school teachers and cadres in Chongqing was investigated by cross-sectional survey. The questionnaires included questionnaires, height, weight, waist circumference, hip circumference, blood pressure, abdominal examination and B Examine and so on. Results Totally 2457 cases of fatty liver were detected, the prevalence of fatty liver was 17.38%, the prevalence of male was 19.67%, the prevalence of female was 13.73%, the prevalence of fatty liver in male was significantly higher than that of female (χ2 = 14.31, P <0.001). The prevalence of fatty liver increased with age (χ2 = 60.97, P <0.0001). The prevalence of fatty liver among different occupational groups was 23.58%, 20.94% of cadres, 19.21% of workers and 6.32% of peasants. The peasant group was the lowest with a very significant difference from the teachers, cadres and workers in the city (χ2 = 70.16, P <0.0001). BMI, abdominal obesity, hypertension and diabetes were closely related to fatty liver by single factor and Logistic multiple regression analysis. The relative risk of BMI and abdominal obesity were the highest (OR = 3.0984 and 3.0305 respectively). Conclusion The prevalence of fatty liver in Chongqing is 17.38%, which is very close to that of Shanghai. The prevalence of fatty liver in peasants was significantly lower than that of urban teachers, cadres and workers. Obesity is the most important risk factor for fatty liver.
