目的研究白细胞和C-反应蛋白与临终患者深部念珠菌感染的关系。方法选取2013年6月-2016年6月医院收治的确诊为深部念珠菌感染的临终患者233例及同期住院患者200例作为研究对象,根据微生物检测结果分成四组,单纯细菌感染组100例、单纯念珠菌感染组101例、念珠菌联合细菌感染组132例和微生物阴性组100例,采用免疫透射比浊法测定C-反应蛋白水平,血细胞分析仪法测定白细胞,比较各组间C-反应蛋白与白细胞水平的差异。结果 233例深部念珠菌感染患者感染以白假丝酵母为主;单纯细菌感染组、单纯念珠菌感染组和念珠菌联合细菌感染组WBC、CRP水平均明显高于微生物阴性组,与单纯细菌感染组比,单纯念珠菌感染组和念珠菌联合细菌感染组WBC、CRP水平均明显升高(P<0.05);与单纯念珠菌感染组相比,念珠菌联合细菌感染组WBC、CRP水平均明显降低(P<0.05);治疗7d后,单纯念珠菌感染组和念珠菌联合细菌感染组WBC及CRP水平明显升高(P<0.05),与单纯念珠菌感染组相比,念珠菌联合细菌感染组WBC差异无统计学意义,CRP水平低于单纯念珠菌感染组(P<0.05);相关性分析结果显示,CRP水平与深部念珠菌感染程度呈正相关,WBC水平与临终患者深部念珠菌感染病情相关性不强。结论 C-反应蛋白水平与深部念珠菌感染病情程度呈正相关,可以用于念珠菌感染病情预测。
Objective To study the relationship between leucocyte and C-reactive protein and deep Candida infection in patients with terminal. Methods From June 2013 to June 2016, 233 cases of dying patients diagnosed as deep Candida infection in hospital and 200 cases of hospitalized patients in the same period were selected as research objects. According to the results of microbiological tests, the patients were divided into four groups, 100 cases of simple bacterial infection group, Candida albicans group 101 cases, Candida combined bacterial infection group 132 cases and microbiological negative group 100 cases, the immunoturbidimetric method was used to measure the level of C-reactive protein, hematology analyzer was used to determine the number of white blood cells, C-reaction between groups was compared Differences in protein and leukocyte levels. Results 233 patients with Candida albicans infection were mainly Candida albicans. The WBC and CRP levels in the group of pure bacterial infection, Candida albicans infection and Candida combined bacterial infection were significantly higher than those in the negative group and pure bacterial infection (P <0.05). Compared with Candida albicans group, the WBC and CRP levels of Candida albicans and bacterial infection group were significantly higher than those of Candida albicans alone group (P <0.05). After 7 days of treatment, the levels of WBC and CRP in the group of Candida infection and Candida combined with bacterial infection were significantly increased (P <0.05). Compared with Candida alone group, Candida combined with bacterial infection There was no significant difference in WBC between groups and the level of CRP was lower than that of Candida alone (P <0.05). Correlation analysis showed that the level of CRP was positively correlated with the degree of deep Candida infection, and the WBC level was closely related to the deep Candida infection Correlation is not strong. Conclusion The level of C-reactive protein is positively correlated with the severity of candidiasis in deep area and can be used for the prediction of Candida infection.
