目的监测下肢深静脉血栓形成(DVT)的溶栓治疗过程中血浆D-二聚体的动态变化。方法用胶体金免疫渗透试验(GIA)法测定24例DVT患者在尿激酶溶栓治疗过程中不同时间(治疗前、治疗后第1、3天、1周、2周、治疗结束后)的D-二聚体含量,同步测定患肢与健侧肢体周径差。结果溶栓治疗后D-二聚体水平显著性升高,峰值出现时间平均为1.67天。治疗后1周D-二聚体下降至溶栓前水平,治疗后2周D-二聚体与溶栓前比较差异有统计学意义。治疗结束后D-二聚体降至正常范围。D-二聚体的变化情况与双下肢周径差的下降相符。结论D-二聚体可以作为DVT溶栓治疗时评价疗效及判断预后的特异性指标。
Objective To monitor the dynamic changes of plasma D-dimer during thrombolytic therapy of deep venous thrombosis (DVT) in the lower extremities. Methods The serum levels of D in 24 patients with DVT were measured by GIA at different time points (before treatment, 1st, 3rd, 1st, 2nd, and after treatment) in the course of urokinase thrombolytic therapy - dimer content, synchronous determination of limb and healthy limb circumference difference. Results The level of D-dimer increased significantly after thrombolytic therapy, with an average peak time of 1.67 days. One week after treatment, D-dimer decreased to the level before thrombolytic therapy. There was significant difference between two groups before D-dimer and before thrombolysis. D-dimer after treatment fell to the normal range. D-dimer changes consistent with the decline in the lower limb circumference difference. Conclusion D-dimer can be used as a specific index to evaluate the efficacy and prognosis of DVT thrombolytic therapy.
