目的探索垂体腺瘤致视力障碍患者视觉皮层及相关功能网络的神经可塑性。方法以25例垂体腺瘤致视力障碍患者及25例与之年龄、性别相匹配矫正后视力正常患者为研究对象,进行神经眼科学检查及结构态和静息态功能核磁扫描。分别对两组受试者静息状态视觉皮层及相关功能网络进行局部一致性分析后,再采用双样本t检验进行统计学分析。结果与正常人群受试者相比,垂体瘤所致视力障碍患者组双侧V1、V2、V3、枕叶外侧皮层,梭状回、MT+以及丘脑局部一致性显著降低,而双侧楔前回、前额叶、扣带回后部皮层、岛叶、缘上回、辅助运动区局部一致性升高。结论垂体腺瘤致前视路功能障碍可引起患者视觉皮层内部脑区神经活动减弱,视觉皮层外默认网络及显著网络内重要脑区神经活动增强。
Objective To explore the neuroplasticity of visual cortex and related functional networks in patients with visual impairment caused by pituitary adenoma. Methods 25 patients with visual impairment caused by pituitary adenoma and 25 age-and-sex matched patients with corrected visual acuity were enrolled in this study. The neuro-ophthalmological examination and structural and resting functional magnetic resonance imaging were performed. After analyzing the local consistency of the resting visual cortex and related functional networks in both groups, the two-sample t-test was used for statistical analysis. Results Compared with normal subjects, the local consistency of V1, V2, V3, occipital lobe cortex, fusiform gyrus, MT + and thalamus in patients with visual impairment caused by pituitary tumor was significantly lower than that in normal subjects. However, Prefrontal cortex, cingulate gyrus, posterior cingulate, marginal gyrus, ancillary motor area increased local consistency. Conclusions The anterior path dysfunction caused by pituitary adenoma can lead to weakened neural activity in the inner cerebral cortex of patients with visual cortex.
