目的 介绍尺动脉腕上皮支上行支皮瓣游离移植修复手指末节指腹皮肤软组织缺损及甲床创面的手术效果.方法 采用尺动脉腕上皮支的上行支游离移植修复手指末节皮肤软组织及甲床缺损共15例,皮瓣面积最大4.0 cm×2.0 cm,最小1.5 cm×1.2 cm.皮瓣蒂部血管神经束与受区指动脉及指腹指背静脉吻合.结果 术后皮瓣全部存活.随访6~24个月.皮瓣外形不臃肿,指腹饱满,外观满意,感觉恢复良好,两点分辨觉为6~ 10 mm.结论 尺动脉腕上皮支上行支皮瓣游离移植修复甲床及末节指腹皮肤软组织缺损创面,皮瓣切取方便,血管解剖恒定,受区外观饱满且感觉功能良好,供区隐蔽,是修复手指末节指腹及甲床皮肤软组织缺损创面的可行方法之一.“,”Objective To report the clinical outcomes of transferring the flap based on ascending branch of ulnar artery above-wrist cutaneous branch to repair soft tissue defect ofthe finger pulp and nail bed.Methods The flap pedicled with the ascending branch of ulnar artery above-wrist cutaneous branch was harvested and transplanted to the finger to repair soft tissue defect of the distal finger and nail bed defect in 15 cases.The size of the biggest flap was 4.0 cm × 2.0 cm,while that of the smallest was 1.5 cm× 1.2 cm.The blood vessels of the flaps were anastomosed to the proper digital artery and dorsal cutaneous vein of the finger in an end-to-end fashion.The cutaneous nerve included in the flap was coapted to the proper digital nerve.Results All the flaps completely survived uneventfully.The patients were follow-up for 6 to 24 months.There was no bulkiness of the flaps.The repaired pulps were full.Good appearance and sensory recovery was achieved.Two-point discrimination returned to 6 to 10 mm.Conclusion The flap pedicled with the ascending branch of ulnar artery above-wrist cutaneous branch is easy to harvest,has constant vascular anatomy,harbors a relatively hidden donor site,and restores contour and sensation to the recipient site.Transfer of this flap is a feasible solution for the treatment of injuries of finger pulp and nail bed.
