目的了解宁波地区ESBLs志贺菌的耐药性,为疾病防控提供依据。方法从监测点和医院分离、收集志贺菌;药敏试验采用K-B法;ESBLs表型确证采用纸片法;CTX-M耐药基因采用PCR法;耐药基因分型采用核苷酸序列法。结果药敏试验筛检出93株ESBLs志贺菌,有78株携带CTX-M耐药基因,检出率为83.87%,其中CTX-M-1群36株,占38.71%,CTX-M-9群42株,占45.16%,携带2种耐药基因的志贺菌21株。未检出CTX-M-2群耐药基因。CTX-M-1群测序分型以CTX-M-15型为主;CTX-M-9群则多为CTX-M-14型。结论本地区志贺菌ESBLs株较多,携带CTX-M较高,甚至可同时携带CTX-M的两群耐药基因,显示志贺菌对抗菌药物耐药率较强,CTX-M-15型和CTX-M-14型是主要的流行酶型。B群志贺菌无论是表型的耐药性,还是耐药基因携带情况均高于D群志贺菌,该发现有助于志贺菌耐药规律的研究。
Objective To understand the drug resistance of Shigella ESBLs in Ningbo and to provide basis for disease prevention and control. Methods Isolation and collection of Shigella from the monitoring sites and hospitals; drug sensitive test using KB method; ESBLs phenotype confirmed by disk method; CTX-M resistance gene using PCR method; resistance gene typing using nucleotide sequence method . Results Ninety-three ESBLs-producing Shigella strains were screened by drug-susceptibility test, and 78 strains harboring CTX-M resistance gene were detected. The detection rate was 83.87%, of which 36 strains were CTX-M-1, accounting for 38.71% 42 strains of 9 groups, accounting for 45.16%, and 21 strains of Shigella carrying two kinds of resistance genes. No CTX-M-2 group resistance gene was detected. CTX-M-1 group sequencing type CTX-M-15 type-based; CTX-M-9 group is mostly CTX-M-14 type. Conclusion There are more shigellobacteria ESBLs strains in our area, carrying higher CTX-M and even carrying two groups of drug resistance genes of CTX-M, indicating that Shigella has a strong antibiotic resistance rate. CTX-M-15 Type and CTX-M-14 type is the major type of enzyme. Shigella flexneri B group is higher than that of Shigella D group in both phenotypic resistance and drug-resistant gene carrier, which is helpful to the study on the resistance of Shigella.
