目的评估血红蛋白(Hb)与脑卒中急性期单侧空间忽略(USN)之间的相关性。方法选择2012年6月1日至2014年9月30日在本院住院的经头颅CT或MRI以及病因学确诊为右半球病变的70例前循环缺血性脑卒中患者为研究对象,并且患者处于发病后48 h内的急性期。通过线段删除(LCT)和星形删除试验(SCT)和平分直线法(LBT)对患者进行USN评定。应用Spearman相关和线性回归模型对Hb与USN之间的关系进行评估。结果脑卒中患者Hb 110~160 g/L,平均(141.3±25.8)g/L,平均红细胞容积(MCV)80~97 fl,平均(91.4±18.4)fl。USN检查结果:LCT评分为0~36分,平均(15.5±2.6)分;SCT评分为4~51分,平均(33.5±8.2)分;LBT评分为14.3~90.9分,平均(64.4±14.7)分。Hb与LCT、SCT和LBT均呈负相关(r值分别为-0.35、-0.27和-0.36,P<0.05)。这些相关性不受混杂因素(如年龄、性别、病情严重程度、功能独立性以及血糖等的影响)。线性回归模型分析结果显示,Hb水平越高,USN表现越好,Hb水平每增加1个单位,平均大约有3条线(b=-3.1)和3个星形(b=-3.2)没被删除。MCV水平与LCT、SCT和LBT的相关性无统计学意义(r值分别为-0.089、0.001和0.063,P>0.05)。结论低水平的Hb可能增加脑卒中急性期的USN程度。
Objective To assess the association between hemoglobin (Hb) and unilateral spatial neglect (stroke) in patients with unilateral spatial neglect (USN). Methods Seventy patients with anterior circulation ischemic stroke admitted to our hospital from June 1, 2012 to September 30, 2014 undergoing cranial CT or MRI and etiology were selected as study subjects, Within 48 h after onset of acute phase. Patients were assessed for USN by line segment deletion (LCT) and star deletion test (SCT) and bisect straight line method (LBT). Spearman correlation and linear regression models were used to assess the relationship between Hb and USN. Results Hb 110 ~ 160 g / L, mean (141.3 ± 25.8) g / L, average mean corpuscular volume (MCV) 80 ~ 97 fl, averaged 91.4 ± 18.4 fl. The results of USN showed that the LCT score ranged from 0 to 36, with an average of (15.5 ± 2.6) points; SCT score ranged from 4 to 51 points (mean, 33.5 ± 8.2); LBT score ranged from 14.3 to 90.9, with an average of (64.4 ± 14.7) Minute. Hb was negatively correlated with LCT, SCT and LBT (r values were -0.35, -0.27 and -0.36 respectively, P <0.05). These correlations are independent of confounding factors (such as age, gender, severity of illness, functional independence, and blood sugar). The results of the linear regression model showed that the higher the Hb level, the better the USN performance. Each additional Hb level had an average of about 3 lines (b = -3.1) and 3 stars (b = -3.2) delete. The correlation between MCV level and LCT, SCT and LBT was not statistically significant (r = -0.089, 0.001 and 0.063 respectively, P> 0.05). Conclusions Low levels of Hb may increase the degree of USN in the acute stage of stroke.
