目的探讨两种不同超声乳化白内障手术切口位置对白内障患者术后角膜散光的影响。方法选择我院超声乳化白内障手术患者81例(81眼),随机分组,对照组40例(40眼),观察组41例(41眼)。对照组予以右上方透明角膜切口手术治疗,观察组实施颞侧透明角膜切口手术治疗,观察比较两组术后角膜散光及视力变化情况,并统计术后并发症发生情况及生活质量评分。结果术后7 d、1个月及2个月观察组角膜散光度均小于对照组,差异有统计学意义(P<0.05);术后2个月内,观察组顺规性散光所占比例较术前增加,逆规性散光较术前减少;术后7 d及术后1个月、2个月观察组矫正视力及裸眼视力均高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为9.76%,对照组为15.00%,组间比较差异无统计学意义(P>0.05);术后观察组生活质量评分高于对照组,差异有统计学意义(P<0.05)。结论超声乳化白内障手术经颞侧透明角膜切口与右上方透明角膜切口比较,效果更优,可有效改善患者术后角膜散光及视力,提高患者生活质量,且安全性高。
Objective To investigate the effect of two different phacoemulsification cataract surgery positions on corneal astigmatism after cataract surgery. Methods 81 patients (81 eyes) with phacoemulsification cataract were randomly divided into control group (40 eyes) and observation group (41 eyes). The control group was treated with clear corneal incision at the upper right, and the observation group was treated with temporal corneal incision. Corneal astigmatism and visual acuity were observed and compared between the two groups. The postoperative complication and quality of life were also evaluated. Results The corneal astigmatism in the observation group at 7 days, 1 month and 2 months after operation was significantly lower than that in the control group (P <0.05). Within 2 months after operation, the percentage of astigmatism Compared with the preoperative, the presbyopic astigmatism decreased significantly. The corrected visual acuity and uncorrected visual acuity of the observation group at 7 days and 1 month and 2 months after operation were significantly higher than those of the control group (P <0.05 ). The incidence of complication in the observation group was 9.76% and in the control group was 15.00%, there was no significant difference between the two groups (P> 0.05). The postoperative quality of life score of the observation group was higher than that of the control group P <0.05). Conclusion The phacoemulsification cataract surgery by the temporal clear corneal incision with the right upper clear corneal incision, the effect is better, which can effectively improve postoperative corneal astigmatism and visual acuity, improve patient quality of life, and high safety.
