目的探讨磁敏感加权成像(SWI)在骨骼肌内血管瘤的诊断及鉴别诊断中的应用价值。方法回顾性分析病理证实的20例骨骼肌内血管瘤的常规MRI及SWI影像。观察肿瘤内部磁敏感信号强度(ITSS)形态、分级、分布及所占肿瘤体积比例,分析肌内血管瘤的SWI特征及鉴别诊断。结果本研究患者肌内血管瘤20例。ITSS形态:点片状4例,团状6例,蔓状10例;ITSS分级:血管瘤20例均为3级;ITSS分布:血管瘤20例均为弥漫性分布;ITSS所占肿瘤体积比例:1/3~2/3组6例,≥2/3组14例。结论肌内血管瘤在SWI序列中表现出独特的影像特征,SWI的应用有助于肌内血管瘤与其他软组织内肿瘤的鉴别诊断。
Objective To investigate the value of magnetic susceptibility weighted imaging (SWI) in the diagnosis and differential diagnosis of skeletal intramuscular hemangiomas. Methods The MRI and SWI images of 20 skeletal intramuscular hemangiomas confirmed by pathology were retrospectively analyzed. The shape, grading, distribution and the proportion of tumor volume in the tumor were observed. The SWI characteristics and differential diagnosis of intramuscular hemangioma were analyzed. Results In this study, 20 patients had intramuscular hemangiomas. The shape of ITSS was patchy in 4 cases, clumpy in 6 cases and clumsy in 10 cases. The ITSS grade was grade 3 in 20 cases of hemangiomas. The distribution of ITSS was diffuse in 20 cases of hemangiomas. The proportion of ITSS in tumor volume : 1/3 ~ 2/3 group of 6 cases, ≥ 2/3 group of 14 cases. Conclusion Intramuscular hemangiomas have unique imaging features in SWI sequences. The application of SWI is helpful for the differential diagnosis between intramuscular hemangiomas and other soft tissue tumors.
