目的探讨65岁及以上老年人中体质指数与血压和血脂的关系。方法资料来源于青岛市城阳区2009~2014年基本公共卫生服务项目管理系统中的老年人查体数据,将系统中的最后一次查体结果作为分析指标,以30 979名信息完整的老年人作为研究对象。将研究对象按体质指数不同分为正常体重组、超重组和肥胖组,采用单因素Logistic回归及多因素Logistic回归分析体质指数和血压、血脂的关系。结果BMI正常组高血压、高血脂的患病率分别为60.50%和53.98%,超重组分别为68.80%和63.37%,肥胖组分别为74.79%和67.33%。超重组和肥胖组高血压、高血脂的患病率明显高于正常组,差异有统计学意义(P<0.01)。单因素Logistic回归分析显示,超重组发生高血压和高血脂的风险分别是正常体重组的1.44(1.37~1.51)倍和1.47(1.40~1.55)倍,肥胖组发生高血压和高血脂的风险分别是正常体重组的1.94(1.79~2.10)倍和1.76(1.63~1.89)倍;多因素Logistic回归调整混杂因素后,超重组发生高血压和高血脂的风险分别是正常体重组的1.43(1.36~1.51)倍和1.33(1.27~1.40)倍,肥胖组发生高血压和高血脂的风险分别是正常体重组的1.91(1.77~2.08)倍和1.40(1.30~1.51)倍。结论超重、肥胖与老年高血压、高血脂有关。
Objective To investigate the relationship between body mass index (BMI) and blood pressure and blood lipids in the elderly 65 years old and above. Methods The data were obtained from the physical examination data of the elderly in the basic public health service project management system in Chengyang District of Qingdao City from 2009 to 2014. The final physical examination results in the system were used as the analysis index to analyze the data of 30 979 complete seniors As research target. Subjects were divided into normal weight group, overweight group and obesity group according to different body mass index. Logistic regression and multivariate Logistic regression were used to analyze the relationship between body mass index and blood pressure and blood lipid. Results The prevalence rates of hypertension and hyperlipidemia in the normal BMI group were 60.50% and 53.98% respectively, while those in the overweight group were 68.80% and 63.37% respectively, while those in the obesity group were 74.79% and 67.33% respectively. The prevalence of hypertension and hyperlipidemia in overweight and obesity group was significantly higher than that in normal group (P <0.01). Univariate logistic regression analysis showed that the risk of developing hypertension and hyperlipidemia in overweight group were 1.44 (1.37-1.51) times and 1.47 (1.40-1.55) times higher than those in normal weight group respectively. The risk of developing hypertension and hyperlipidemia in obesity group were Was 1.94 (1.79 ~ 2.10) times and 1.76 (1.63 ~ 1.89) times higher than that of normal weight group. The risk of hypertension and hyperlipemia in overweight group were 1.43 (1.36 ~ 1.51) and 1.33 (1.27-1.40) times, respectively. The risk of developing hypertension and hyperlipidemia in obesity group were 1.91 (1.77 ~ 2.08) times and 1.40 (1.30 ~ 1.51) times of normal weight group, respectively. Conclusion Overweight and obesity are associated with hypertension and hyperlipemia in the elderly.
