目的探讨单纯性肥胖儿童体内中性粒细胞明胶酶相关脂质运载蛋白(NGAL)的变化及意义。方法采用整群抽样法抽取7~18岁肥胖儿童90例,按体质量指数(BMI)将其分为轻、中、重度肥胖组,每组30例。同时以正常体重儿童30例作为健康对照组。对所有受试者测定空腹血糖(FBG)、血NGAL等相关指标水平,并测定胰岛素抵抗指数(HOMA-IR)。结果三组肥胖儿童FBG与健康对照组比较,差异无统计学意义(P>0.05);轻度肥胖组儿童体内NGAL及HOMA-IR与健康对照组相比,差异无统计学意义(P>0.05);中、重度肥胖组儿童体内NGAL及HOMA-IR均高于健康对照组,差异有统计学意义(P<0.05)。结论明显的肥胖可影响儿童体内中性粒细胞明胶酶相关脂质运载蛋白、胰岛素等激素水平的分泌,是其日后出现胰岛素抵抗、甚至发展为糖尿病的可能原因。
Objective To investigate the changes and significance of neutrophil gelatinase-associated lipocalin (NGAL) in simple obese children. Methods A total of 90 obese children aged 7-18 years were enrolled in this study. According to body mass index (BMI), they were divided into mild, moderate and severe obesity group, 30 cases in each group. At the same time, 30 healthy children were used as healthy control group. All subjects were measured fasting blood glucose (FBG), blood NGAL and other related indicators, and determination of insulin resistance index (HOMA-IR). Results There was no significant difference in FBG between the three groups of obese children and the healthy control group (P> 0.05). There was no significant difference in NGAL and HOMA-IR between the mild obesity group and the healthy control group (P> 0.05 ). The levels of NGAL and HOMA-IR in children with moderate and severe obesity were significantly higher than those in healthy controls (P <0.05). Conclusion Obvious obesity can affect the secretion of gelatinase-related lipocalin, insulin and other hormone levels in children, which is the possible cause of insulin resistance and even development of diabetes.
