目的应用神经内镜经颞下锁孔入路对上岩斜区进行局部解剖学研究,探索临床经此入路到达该区域的可行性。方法用15例福尔马林固定的成人尸头、1例新鲜成人尸头,显微镜和内镜(0°镜、30°镜)交替配合使用,经PcoA—CNⅢ间隙、PCA-丘脑前穿支间隙、PcoA穿通支间隙、CNⅢ-天幕缘间隙,观察上岩斜区。结果上述间隙中,PcoA-CNⅢ间隙观察范围最广泛,常与PCA-丘脑前穿支间隙、PcoA穿通支间隙配合使用,用于观察第三脑室底和脚间窝。结论颞下锁孔入路能减少对周围结构的侵袭,而内镜对上岩斜区解剖学研究为临床手术提供了重要的可参考信息。
Objective To study the local anatomy of the upper petroclival region by means of endoscopic ultrasonography and endoscopic approach to explore the feasibility of this clinical approach to this region. Methods 15 formalin-fixed adult cadaver heads, 1 fresh adult cadaver head, microscope and endoscope (0 ° mirror, 30 ° mirror) were used in alternation. The PcoA-CNⅢ gap, PCA- Gap, PcoA through the branch gap, CN Ⅲ-day gap edge, observe the upper rock slope. Results The above-mentioned gap, PcoA-CN Ⅲ clearance the most extensive range of observation, often with PCA-thalamic perforating space, PcoA through the gap with the use of the branch for the observation of the third ventricle and foot fossa. Conclusions Temporo-inferior keyhole approach can reduce the invasion of the surrounding structures. The anatomic study of the upper petroclival region by endoscopy provides important information for clinical operation.
