Objective: Study based on extra levator wide resection of cancer lower third of the rectum through perineal approach; avoided opening of the abdomen, abdominal colostomy and potential dangers of second stage prone position during abdominoperineal cylindrical resection of the rectum.It had assed reduction of morbidity of opening abdomen and supine position.Patients & Method: During the period between Jan.2006 to Dec.2011, 32 cases adenocarcinoma of distal third of rectum staged T2 , assed by MRI, Endosnography.All cases were required abdominoperineal resection with permanent colostomy.They were given adjuvant chemotherapy.In lymph nodes positive cases, postoperative chemotherapy was given.Median follow up was of 48 months from 24 months to 60 months.Results: The complete removal of pathology was achieved in all cases local recurrence was noted in 3 cases.No colostomy was required, continence was present in 24 out of 32 cases, in 6 cases night enema was required and pads in 2 cases.Postoperative period was comfortable due to not opening of the abdomen.Conclusion: The perineal approach for the cylendrical resection of lower third ca rectum is simple, eradicates disease effectively without opening of the abdomen.The patients were given next day oral liquid.It reduces morbidity of the supine position during the surgery.
