Reconstruction of wounds involving exposed tendons and/or bones is can be challenging because tendons and bones have poor blood circulation.We describe our experiences with the perifascial areolar tissue (PAT) graft with topical fibroblast growth factor (FGF) therapy and a dermal substitute to treat 8 patients with complex extremity wounds that involved exposed tendons and/or bones and were caused by trauma or burns.All grafts showed good survival and successfully covered tendons and bones.Seven patients subsequently received split-thickness skin grafts, and 1 patient experienced epithelialization without surgical treatment.The PAT graft is minimally invasive and is suitable for treating irregular defects.PAT graft placement is a technically easy surgical procedure and the donor site is relatively preserved.However, the PAT graft sometimes failed to survive because of the associated poor blood circulation.We recommend the PAT graft with topical FGF therapy and dermal substitute for treating complex extremity wounds involving exposed tendons and/or bones.
