Preoperative 5-fluorouracil-based chemoradiotherapy (CRT) and total mesorectal excision surgery have led to a reduction in local recurrence rates being nowadays less than 10%.But despite this great information, the development of distant metastases goes on being the most frequent location of failure (25-30%).Newer generation chemotherapeutics,such as oral fluoropyrimidines, oxaliplatin, irinotecan, and targeted therapies, such as bevacizumab and cetuximab,have been incorporated into phase Ⅰ-Ⅱ studies with preoperative RT for rectal cancer but phase Ⅲ trials are still needed to determine if these drugs will offer an advantage compared with 5-fluorouracil-based combined modality regimen.Early results from the ACCORD 12/0405-Prodige 2 and STAR-01 trial did not confirm a significant improvement of important endpoints, such as the pCR rate, with the addition of oxaliplatin to preoperative 5-fluorouracil based CRT,although long-term results are awaited.Thus, the neoadjuvant treatment for rectal cancer is still challenging and according to several aspects it is controversial.We have carried out a project to clear up these points.
