Objective: Parallels between Parkinsons disease (PD) and REM sleep behavior disorder (RBD) have been reported for many years.According to ICSD-Ⅱ, RBD had lost of REM sleep muscle atonia (RSWA).Some PD patients without RBD had RSWA.The aim of this study was to exam relationships between the degree of RSWA and clinical characteristics in PD patients.Methods: We conducted overnight polysomnography studies in 198 PD patients.Tonic and phasic muscle activity during REM sleep were quantified using EMG recordings from the chin, compared among study groups, and correlated with clinical characteristics.Results: All PD patients were categorized into quartiles of tonic and phasic EMG activity, respectively.We found that the patients with more chin EMG activity were older.After adjusted for age and gender, the patients with more chin EMG activity were longer PD duration, higher H-Y stages, more levodopa-equivalent daily dose(LED), wore cognition and quality standard of life.The LED of highest quartile were highest.In addition, after adjusted for age and gender, the patients with more chin EMG activity were higher percentage of stage 1 sleep and lower percentage of stage 2 sleep.After adjusted for age, gender and PD duration, patients in the highest quartile of chin EMG activity were more likely to have lately PD patients (H&Y scale ≥3.0) than those in the lowest quartile.The OR value of tonic and phasic EMG activity were 13.2 (95%CI: 2.6-67.5, P=0.002) and 6.9 (95%CI: 2.3-21.2, P=0.001), respectively.As level of chin EMG activity, increased, OR of lately PD patients in each quartile positively increased (P for trend=0.001, 0.001).The tonic and phasic EMG activity were observed to be associated with age, PD duration, H-Y stages, LED, the scores of UPDRSⅢ, MoCA and PDQ scores, NREM1 and NREM2.Conclusion: These findings provided evidence that the RSWA was associated with age and severity of illness in PD patients.The chin EMG activity might be a better index that reflected the microstructure performance of PD progression, especially tonic chin EMG.As level of chin EMG activity increased, OR of lately PD patients in each quartile positively increased.
