Conventional orthognathic surgery is composed of three stages: the presurgical orthodontic treatment for dental decompensation followed by the orthognathic surgical procedure, and finally post-surgical orthodontic finishing.This orthognathic surgery has been widely used because of the predictable and successful outcomes.However, the incisor decompensation results in the worsening ofpre-surgical facial profile and dental function.The significant patient discomfort led to a surgery-first approach, which eliminates the presurgical orthodontic treatment.This surgery-first approach has several advantages, such as significant reduction in total treatment time, rapid improvement in facial esthetics, and high patient satisfaction after surgery.Recently, the surgery-first approach can be widely used in most patients with dentofacial deformities with the aid of a skeletal anchorage system and 3D computed tomography.
