Objective To evaluate the effects of an aquatic exercise programme designed to enhance muscular strength in paretic lower limbs without increasing spasticity in subacute stroke patients.Methods Subacute stroke patients 3-6 months after first-ever stroke onset (n =20) were recruited.Patients were randomly assigned to either the aquatic group or the conventional group.Group exercise sessions 40 min five times per week for 4 weeks.Aquatic group (n =10) performed exercises in chest-deep water.Conventional group (n =10)performed land-based therapy.Maximum isometric voluntary contraction (MIVC) of knee extensors and flexors and ankle dorsiflexors and plantar flexors of paretic lower limbs were measured by joint torque and surface EMG.Cocontraction ratios between rectus femoris and bicipitis femoris caput longus, tibialis anterior, and lateral gastrocnemius were calculated.Modified Ashworth Scale (MAS) and Functional Ambulation Category (FAC) were used to assess lower-limb joint spasticity and motor skills necessary for functional ambulation, respectively.
