目的观察人乳头瘤病毒(human papillomavirus,HPV)与宫颈病变的相关性,并分析宫颈病变患者HPV感染的危险因素,为临床防治宫颈病变提供依据。方法以2013~2016年北京市第一中西医结合医院收治的120例宫颈病变和80例正常宫颈人群为研究对象,分析正常宫颈人群和不同类型宫颈病变患者的HPV感染差异,分析不同类型宫颈病变患者的HPV感染类型差异,并对宫颈病变患者HPV感染的危险因素进行分析。结果正常宫颈人群的HPV感染率(21.25%)显著低于宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)Ⅰ型、CINⅡ型、CINⅢ型、宫颈腺癌、宫颈鳞癌及其他病变类型的宫颈病变患者的感染率(P<0.05);CINⅠ型、CINⅡ型、CINⅢ型、宫颈腺癌、宫颈鳞癌及其他病变类型的宫颈病变患者的单一型HPV感染率显著高于混合型HPV感染率(P<0.05);年龄、文化程度和初次性生活年龄是宫颈病变患者HPV感染的独立危险因素。结论各种宫颈病变患者的HPV感染率高于正常人群,且病变程度越严重患者感染率越高,年龄、文化程度和初次性生活年龄是HPV感染的独立危险因素,临床防治宫颈病变应重视HPV对患者病情的影响。
Objective To observe the correlation between human papillomavirus (HPV) and cervical lesions and analyze the risk factors of HPV infection in patients with cervical lesions so as to provide evidence for clinical prevention and treatment of cervical lesions. Methods A total of 120 cases of cervical lesions and 80 normal cervix patients admitted to the First Hospital of Integrated Chinese and Western Medicine in Beijing from 2013 to 2016 were selected to analyze the differences in HPV infection between normal cervical population and different types of cervical lesions and to analyze the relationship between different types of cervical lesions The type of HPV infection in patients with cervical lesions and HPV infection risk factors were analyzed. Results The HPV infection rate in normal cervical population (21.25%) was significantly lower than that in cervical intraepithelial neoplasia (CIN) type Ⅰ, CINⅡ, CINⅢ, cervical adenocarcinoma, cervical squamous cell carcinoma and other types of cervical lesions (P <0.05). The prevalence of single HPV infection in patients with cervical lesions of CINⅠ, CINⅡ, CINⅢ, cervical adenocarcinoma, cervical squamous cell carcinoma and other lesions was significantly higher than that of mixed HPV <0.05). Age, education level and first-time sex life were independent risk factors for HPV infection in patients with cervical lesions. Conclusion The HPV infection rate of all kinds of cervical lesions is higher than that of normal people, and the more severe the disease is, the higher infection rate, age, education level and first-time life are the independent risk factors of HPV infection.
