目的探究并分析子痫前期患者三维能量多普勒超声(3D-PDU)检测指标和胎盘生长因子(PLGF)及可溶性血管内皮细胞生长因子受体-1(s Flt-1)表达的相关性。方法选取2015年9月—2016年9月滁州市第一人民医院收治的100例子痫前期患者,按照子痫严重程度分为轻度组和重度组,每组50例,另选取待产的健康产妇50例,作为对照组。所有孕妇产前均进行3D-PDU检查,并利用三维重建技术和VOCAL软件测量血管化指数(VI)、血流指数(FI)和血管化血流指数(VFI),分娩后检测PLGF及sFlt-1水平,统计并分析3D-PDU检查结果与胎盘PLGF及sFlt-1水平的相关性。结果轻度组以及重度组患者胎盘VI、FI、VFI均明显小于对照组(P<0.05),重度组患者胎盘VI、FI、VFI均明显小于轻度组(P<0.05),随着患者病情加重,胎盘VI、FI、VFI呈下降趋势;轻度组以及重度组患者胎盘PLGF均明显小于对照组(P<0.05),重度组患者胎盘PLGF明显小于轻度组(P<0.05),随着患者病情加重,胎盘PLGF呈下降趋势;轻度组以及重度组患者胎盘sFlt-1均明显大于对照组(P<0.05),重度组患者胎盘sFlt-1明显大于轻度组(P<0.05),随着患者病情加重,胎盘SFlt-1呈上升趋势,差异具有统计学意义。经皮尔逊相关性分析,胎盘VFI与胎盘PLGF具有显著相关性(P<0.01),两者呈正相关。胎盘VFI与胎盘sFlt-1具有显著相关性(P<0.01),两者呈负相关。结论子痫前期患者3D-PDU检测指标与胎盘PLGF及sFlt-1的表达具有显著相关性,可为子痫前期临床诊断提供一定的帮助。
Objective To investigate and analyze the correlation between the detection of 3D-PDU and the expression of placental growth factor (PLGF) and soluble vascular endothelial growth factor receptor-1 (s Flt-1) in patients with preeclampsia. Methods A total of 100 preeclampsia patients admitted to Chuzhou First People’s Hospital from September 2015 to September 2016 were divided into mild group and severe group according to the severity of eclampsia, 50 cases in each group. The healthy pregnant women 50 cases, as a control group. All pregnant women underwent prenatal 3D-PDU examinations. Vascularization index (VI), blood flow index (FI) and vasculogenic blood flow index (VFI) were measured by three-dimensional reconstruction and VOCAL software. PLGF and sFlt- 1 level, statistics and analysis of 3D-PDU examination results and placental PLGF and sFlt-1 level correlation. Results The levels of VI, FI and VFI of placenta in mild and severe group were significantly lower than those in control group (P <0.05). The levels of VI, FI and VFI in severe group were significantly lower than those in mild group (P <0.05) (P <0.05). PLGF in placenta in severe group was significantly lower than that in mild group (P <0.05). With the increase of placenta, FI, VFI, the placental PLGF in mild group and severe group was significantly lower than that in control group Placental sFlt-1 levels in mild and severe groups were significantly higher than those in control group (P <0.05). Placental sFlt-1 levels in severe group were significantly higher than those in mild group (P <0.05) As the patient exacerbated, placental SFlt-1 showed an upward trend, the difference was statistically significant. By Pearson correlation analysis, there was a significant correlation between placental VFI and placental PLGF (P <0.01), and there was a positive correlation between them. There was a significant correlation between placental VFI and placental sFlt-1 (P <0.01), both of them were negatively correlated. Conclusion The detection of 3D-PD in preeclampsia patients has a significant correlation with the expression of PLGF and sFlt-1 in placenta, which may provide some help for the clinical diagnosis of preeclampsia.
