目的描述社区脑梗死患者住院医疗费用和住院天数的集中趋势及构成比,并探索影响医疗费用的主要因素,为降低脑梗死患者医疗费用提供政策性依据。方法以唐山市缸窑街道办事处社区卫生服务中心脑梗死患者医疗费用为研究对象,测算出医疗费用的中位数、P25、P75及费用构成比,利用多元线性逐步回归探索影响患者医疗费用的因素。结果医疗总费用中位数为4 716.57元,住院天数中位数为16d,医疗费用构成比为:药费(64.28%)、检查费(10.80%)、检验费(10.06%)治疗费(8.25)、床位费(4.55)、其他费用(2.24%),影响医疗费用的因素为患者住院天数、并发症种类和医生。结论住院天数、并发症数量和种类、主诊医生是影响住院费用的主要因素,应针对其中可控危险因素采取必要的干预措施降低住院费用,尤其是药品费用。
Objective To describe the central tendency and composition ratio of hospitalization expenses and days of hospitalization for patients with cerebral infarction in community and to explore the main factors influencing medical expenses and provide a policy basis for reducing the medical expenses for patients with cerebral infarction. Methods Taking the medical expenses of patients with cerebral infarction in Community Health Service Center of Tangyao Kiln Sub-district Office as the research object, the median of medical expenses, P25 and P75 and the cost composition ratio were calculated, and the multivariate linear stepwise regression was used to explore the influencing factors factor. Results The median total cost of medical treatment was 4 716.57 yuan and the median number of days of hospitalization was 16 days. The cost of medical treatment was 64.28%, the examination fee was 10.80%, the examination fee was 10.06% ), Bed fee (4.55) and other expenses (2.24%). The factors influencing medical expenses are hospital stay days, types of complications and doctors. Conclusions The days of hospitalization, the number and types of complications, the chief doctor is the main factor affecting hospitalization costs, and the necessary interventions should be taken for the controllable risk factors to reduce hospitalization expenses, especially the cost of medicines.
