目的:了解某三甲综合医院内分泌专科首次会诊疾病谱的构成特点。方法:回顾性分析2011年6月1日至2014年6月30日广州医科大学附属第一医院内分泌专科首次会诊病例的临床资料,分析其疾病构成特点。结果:纳入研究病例4 081例,男1 970例(48.3%),女2111例(51.7%);年龄4~97岁,中位年龄64岁,其中65岁以上2 128例(52.1%);非手术科室2 603例(63.8%)、手术科室1 478例(36.2%)。疾病构成类别前3位依次是糖代谢异常2 938例(72.0%)、甲状腺功能异常846例(20.7%)、电解质紊乱119例(2.9%)。邀请频次前3位依次是呼吸内科792例(19.4%)、心血管内科501例(12.3%)、胸外科398例(9.8%)。结论:合并内分泌科疾病且需要专科指导的患者以糖代谢异常及甲状腺功能异常多见,且广泛分布于各科室,尤其在非手术科室更常见,应针对性地加强专科医生的诊治能力。
Objective: To understand the composition characteristics of the first consultation disease spectrum of endocrine specialist in a top three general hospital. Methods: The clinical data of the first consultation of endocrine specialist in the First Affiliated Hospital of Guangzhou Medical University from June 1, 2011 to June 30, 2014 were retrospectively analyzed, and the characteristics of the disease were analyzed. Results: A total of 4 081 cases were included in the study, including 1 970 (48.3%) male and 2111 (51.7%) female patients. The median age was 64 years (range, 4-97 years) There were 2 603 cases (63.8%) of non-surgical departments and 1 478 cases (36.2%) of surgical departments. The top 3 diseases were 2938 (72.0%) with abnormal glucose metabolism, 846 (20.7%) with abnormal thyroid function, and 119 (2.9%) with electrolyte imbalance. Among the top three invitations, 792 (19.4%) were respiratory medicine, 501 (12.3%) were cardiovascular medicine and 398 (9.8%) were thoracic surgery. CONCLUSIONS: Patients with endocrine diseases who require specialist guidance are more common in patients with abnormal glucose metabolism and thyroid function. They are widely distributed in various departments and departments, especially in non-surgical departments. The diagnosis and treatment of specialists should be strengthened in a targeted manner.
