目的:探讨pSS住院患者继发血细胞减少和间质性肺病(ILD)的相关因素。方法:回顾性分析2014年3月至2020年10月于中日友好医院住院pSS患者的临床特点,根据是否继发血细胞减少和ILD进行分组,采用n χ2检验和Mann-Whitney n U检验进行组间比较,Logistic回归探索pSS继发血细胞减少和ILD的相关因素。n 结果:共纳入pSS住院患者571例,男女比例约1∶8,存在血细胞减少275例(48.2%)、ILD 180例(31.5%)。pSS继发血细胞减少较无血细胞减少患者C3降低[n OR=2.326,95%n CI(1.483,3.650),n P<0.01]和ESR升高[n OR=1.912,95%n CI(1.233,2.964),n P<0.01]的比例更高。pSS继发ILD较无ILD患者男性[n OR=2.509,95%n CI(1.269,4.959),n P<0.01]和乏力[n OR=5.190,95%n CI(5.190,13.931),n P<0.01]的比例更高,但抗SSA抗体阳性[n OR=0.392,95%n CI(0.230,0.668),n P<0.01]、抗着丝点蛋白B(CENPB)抗体阳性[n OR=0.337,95%n CI(0.145,0.782),n P<0.01]及低C3[n OR=0.332,95%n CI(0.189,0.582),n P<0.01]的比例更低。n 结论:低C3和ESR升高可能是pSS继发血细胞减少的危险因素;男性pSS患者可能更易继发ILD,而pSS继发ILD患者抗SSA抗体、抗CENPB抗体阳性及低C3的比例可能更低,有待前瞻性队列研究验证。“,”Objective:To detect the factors related to hemocytopenia and interstitial lung disease (ILD) secondary to primary Sj?gren's syndrome (pSS) in in-patients.Methods:Clinical characteristics of patients with pSS hospitalized in China-Japan Friendship Hospital from March 2014 to October 2020 were retrospectively analyzed. Patients were divided into different groups according to hemocytopenia and ILD. Chi-square test and Mann-Whitney n U test were used to compare data in subgroups, and logistic regression were used to detect factors related to hemocytopenia and ILD secondary to pSS.n Results:Five hundred and seventy-one inpatients with pSS were included in this study and the female: male ratio was 8∶1. Two hundred and seventy five of included patients had hemocytopenia (48.2%) and 180 patients had ILD (31.5%). Compared with patients without hemocytopenia, patients with hemocytopenia had higher ratio in low C3 [n OR=2.326, 95%n CI(1.483, 3.650), n P<0.01] and elevation of erythrocyte sedimentation rate (ESR) [n OR=1.912, 95%n CI(1.233, 2.964), n PP<0.01) . Compared with patients without ILD, patients with ILD showed higher ratio in male[n OR=2.509, 95%n CI(1.269, 4.959), n P<0.01) and fatigue [n OR=5.190, 95%n CI(5.190, 13.931), n P<0.01) , lower positive rate of anti-SSA antibody [n OR=0.392, 95%n CI(0.230, 0.668), n P<0.01) and anti-CENPB [n OR=0.337, 95%n CI(0.145, 0.782), n P<0.01] antibodies, and lower ratio in low C3[n OR=0.332, 95%n CI(0.189, 0.582), n P<0.01].n Conclusion:Low C3 and high ESR may be risk factors for developing hemocytopenia in patients with pSS. Men with pSS may be susceptible to ILD, while pSS patients with ILD may have lower ratio in positive anti-SSA antibody, positive anti-CENPB antibody and low C3. All of these findings are yet to be validated by future prospective cohort studies.
