目的探讨不同程度的妊高征对早产儿体格生长与神经运动发育的影响。方法 2009年1月至2010年12月在中山市博爱医院高危儿随访中心建立档案,母亲有妊高征高危因素的早产儿,分为三组(轻度妊高征组;中度妊高征组;重度妊高征组)。前瞻性的对其进行跟踪随访至纠正月龄6个月,测量身高,体重,头围和智力发育情况。结果①三组间出生时体重、身长、头围比较:轻度妊高征组与中、重度妊高征组差别有统计学意义。②纠正胎龄40周时,3组间身长、头围两个指标均为轻度妊高征组与中重度妊高征组差别有统计学意义。③在纠正6月龄时,三组的体重、身长、头围的差别无统计学差异。④生后纠正胎龄40周时行NBNA评分,不同妊高征程度组间的差别无统计学差异。⑤生后纠正6月龄时行Gesell检查,轻度妊高征组与中重度妊高征组差别有统计学差异。结论不同程度妊高征对早产儿早期的体格发育影响不大,对神经运动发育有影响,轻度影响较小,中重度影响较大。在孕期应严格控制轻度妊高征发展成中重度妊高征,减少对婴儿神经运动发育的影响。
Objective To investigate the effects of different degrees of pregnancy-induced hypertension on the growth of premature infants and the development of neural motor. Methods From January 2009 to December 2010, a file was set up at the Follow-up Center of High-risk Children in Pok Oi Hospital of Zhongshan City. The mothers were premature infants with high risk of PIH. They were divided into three groups (mild PIH group, moderate PIH Group; severe PIH group). They were followed up prospectively until 6 months of age, measuring height, weight, head circumference and mental development. Results ①The birth weight, length and head circumference of the three groups were compared: there was significant difference between mild PIH group and moderate and severe PIH group. ② Correct gestational age 40 weeks, the three groups of length, head circumference of the two indicators are mild pregnancy-induced hypertension group and moderate to severe pregnancy-induced hypertension group differences were statistically significant. ③ Correction of 6-month-old, the three groups of weight, length, head circumference difference was no significant difference. ④ after birth to correct gestational age 40 weeks NBNA score, the difference between the different groups of pregnancy-induced hypertension was no significant difference. ⑤ After birth correction Gesell examination at 6 months of age, mild PIH group and moderate to severe PIH differences were statistically significant. Conclusion Different degrees of pregnancy-induced hypertension have little effect on the early physical development of premature infants, and have an impact on the development of neuro-motor, with mild and moderate effects. Should be strictly controlled during pregnancy, mild PIH developed into moderate and severe pregnancy-induced hypertension, reduce the impact of infant neurological development.
