目的:探讨代谢综合征(MS)并发冠心病(CHD)的患者空腹血浆P-选择素水平和胰岛素敏感性、氧化修饰型低密度脂蛋白(ox-LDL)及MS其他指标的相互关系。方法:用液相蛋白芯片结合流式细胞仪测定单纯MS组(7例)、MS并CHD组(33例)和正常对照组(9例)空腹血浆P-选择素水平,用酶联免疫吸附法(ELISA)测定空腹血浆胰岛素、ox-LDL水平,并收集MS的其他有关数据:腰围、体质指数、空腹血糖和血脂,计算胰岛素敏感指数(ISI)。结果:①P-选择素在正常对照组、MS组、MS并CHD组分别为(25.3±3.5)、(32.8±5.1)、(40.4±8.0)μg/L,3组间差异有统计学意义,(F=17.25,P<0.05);②空腹血浆P-选择素水平与ISI呈负相关(r=-0.478,P<0.05),与ox-LDL(r=0.342,P<0.05)、体质指数(r=0.326,P<0.05)、LDL-C(r=0.336,P<0.05)、腰围(r=0.500,P<0.05)、空腹胰岛素(r=0.467,P<0.05)呈正相关;③以空腹血浆P-选择素水平为应变量的多元逐步回归分析提示,ISI、腰围、空腹真胰岛素、LDL、ox-LDL与P-选择素具有相关性(均P<0.05)。结论:MS患者P-选择素水平增加,MS并CHD的患者P-选择素增高更为明显,并与胰岛素敏感性下降、ox-LDL升高相关,可能在MS发展为CHD中起一定作用。
Objective: To investigate the relationship between fasting plasma P-selectin level and insulin sensitivity, ox-LDL and other MS in patients with metabolic syndrome (MS) complicated with coronary heart disease (CHD). Methods: Fasting plasma P-selectin level was measured by liquid protein chip combined with flow cytometry in MS group (MS), MS group (33 cases) and normal control group (9 cases) Fasting plasma insulin and ox-LDL levels were measured by ELISA. Other relevant data of MS were collected: waist circumference, body mass index, fasting blood glucose and lipids, and insulin sensitivity index (ISI) were calculated. Results: ①P-selectin was (25.3 ± 3.5), (32.8 ± 5.1) and (40.4 ± 8.0) μg / L in normal control group, MS group and MS combined with CHD group respectively. There were significant differences among three groups (P <0.05); ②The level of plasma P-selectin was negatively correlated with ISI (r = -0.478, P <0.05) (r = 0.326, P0.05), LDL-C (r = 0.336, P0.05), waist circumference (r = 0.500, P0.05) and fasting insulin (r = 0.467, Multiple stepwise regression analysis of fasting plasma P-selectin level as a function of strain showed that ISI, waist circumference, fasting true insulin, LDL, ox-LDL were correlated with P-selectin (all P <0.05). CONCLUSIONS: Patients with MS have increased P-selectin levels and increased P-selectin levels in patients with MS and CHD, and are associated with decreased insulin sensitivity and elevated ox-LDL, which may play a role in the development of MS in patients with CHD.
