目的:探讨临床护理路径应用于2型糖尿病患者健康教育中的作用。方法:选择苍南县第二人民医院内分泌科2015年7月-2016年12月100例2型糖尿病患者,实施健康教育,按照随机数字表法分成对照组50例,实验组50例。其中对照组给予传统护理及健康教育;实验组采用临床护理路径模式进行护理及健康教育。分别于干预前、干预3个月时检测患者血糖状况(血糖、HbA1c)、调查两组干预前、干预3个月时健康知识掌握和自我管理能力情况。结果:两组患者干预3个月时空腹血糖、餐后2h血糖及糖化血红蛋白(HbA1c)水平较干预前显著下降;实验组干预3个月时空腹血糖、餐后2h血糖及糖化血红蛋白(HbA1c)水平明显低于对照组,P<0.05。两组患者干预3个月时健康知识掌握和自我管理能力评分明显高于干预前,实验组干预3个月时健康知识掌握和自我管理能力评分明显高于对照组,P<0.05。结论:糖尿病健康教育中实施临床护理路径可有助于控制患者血糖水平,提高患者对健康知识的掌握及自我管理能力,减少并发症发生率,提高护理质量。
Objective: To explore the role of clinical nursing pathways in health education of patients with type 2 diabetes mellitus. Methods: 100 cases of type 2 diabetes patients in Cangnan Second People’s Hospital from July 2015 to December 2016 were selected for health education. According to the random number table, 50 cases were divided into control group (50 cases) and experimental group (50 cases). The control group was given traditional nursing and health education. The experimental group adopted the clinical nursing path mode for nursing and health education. The blood glucose level (blood glucose, HbA1c) was measured at 3 months before intervention and 3 months after intervention respectively. The health knowledge and ability of self-management before and 3 months after intervention were investigated. Results: The levels of fasting blood glucose, 2h postprandial blood glucose and HbA1c were significantly decreased at 3 months after intervention in both groups. Fasting plasma glucose, postprandial 2h HbA1c, The level was significantly lower than the control group, P <0.05. The score of health knowledge mastery and self-management ability of the two groups were significantly higher than that of the control group at 3 months after intervention. The scores of mastery of health knowledge and self-management ability at 3 months after intervention in the experimental group were significantly higher than those in the control group (P <0.05). Conclusion: The implementation of clinical nursing pathway in diabetes health education can help to control the blood sugar level of patients, improve their ability to master the health knowledge and self-management ability, reduce the incidence of complications and improve the nursing quality.
