目的检测腹泻型肠易激综合征(IBS)患者升结肠和降结肠黏膜水通道蛋白8(AQP8)的表达情况,研究IBS与AQP8的关系及IBS的发病机制。方法采用荧光定量RT-PCR方法对人结肠黏膜细胞的AQP8mRNA进行定量分析研究,探讨AQP8的表达与IBS的发生及临床特征的关系。结果正常人和IBS患者升结肠和降结肠组织标本中均有AQP8mRNA表达。腹泻型IBS患者升结肠和降结肠的AQP8mRNA表达显著低于正常对照组(中位数3·1×104拷贝/μgRNA和2·8×104拷贝/μgRNA比8·2×104拷贝/μgRNA和3·8×104拷贝/μgRNA),AQP8mRNA在结肠的表达水平与患者年龄和始发病年龄均无相关性,但与病程长短、排便频率及排便性状密切相关,病程越长,排便频率越高,大便含水量越多,其结肠黏膜AQP8mRNA的表达量就越低。结论腹泻型IBS患者AQP8表达下降,提示结肠吸收功能发生障碍,水吸收量减少,导致粪便稀薄形成腹泻。AQP8的变化与IBS的发生可能存在一定相关性。
Objective To detect the expression of aquaporin 8 (AQP8) in ascending colon and descending colon mucosa in patients with irritable bowel syndrome (IBS) and to investigate the relationship between IBS and AQP8 and the pathogenesis of IBS. Methods Quantitative analysis of AQP8 mRNA in human colonic mucosa by fluorescence quantitative RT-PCR and to explore the relationship between the expression of AQP8 and the occurrence and clinical features of IBS. Results AQP8 mRNA expression was detected in ascending colon and descending colon specimens of normal and IBS patients. The expression of AQP8mRNA in ascending colon and descending colon in patients with diarrhea-type IBS was significantly lower than those in the normal controls (median 3 · 1 × 104 copies / μgRNA and 2 · 8 × 104 copies / μgRNA vs. 8 · 2 × 104 copies / μgRNA and 3 · 8 × 104 copies / μg RNA). There was no correlation between AQP8mRNA expression in colon and patient’s age and onset age, but it was closely related to duration of disease, defecation frequency and defecation. The longer the course of disease, the higher the frequency of defecation, Stool water content, the lower the amount of colon mucosal AQP8 mRNA expression. Conclusion The decrease of AQP8 expression in diarrhea-predominant IBS patients suggests that the intestinal absorption function is impaired and the amount of water absorbed is reduced, leading to the formation of thin stools and diarrhea. AQP8 changes and the occurrence of IBS may have some relevance.
