目的:探讨难治性癫痫患儿应用生酮饮食对其滤泡辅助性T细胞及B细胞的影响。方法:将32例难治性癫痫患儿设为实验组,选取正常同龄对照20例,流式细胞术检查生酮饮食治疗前后Tfh细胞及B细胞比例,ELISA检测IL-21血清中浓度,RT-PCR检测Blimp-1、Bcl-6、m TOR基因表达情况。结果:生酮治疗后Tfh细胞数量明显下降,Tfh细胞抑制因子Blimp-1表达降低,转录相关因子Bcl-6表达也降低,血清中IL-21浓度及m RNA表达均显著减低,CD27+Ig D-B细胞、CD27+Ig D-BCD38bigb与治疗后相比显著下降。治疗后Tfh细胞及各阶段B细胞与m TOR表达存在正相关关系。结论:KD治疗通过m TOR降低了Tfh细胞与B细胞比例或是抑制了Tfh细胞与B细胞的功能,这可能是导致接受KD治疗过程中少数患儿出现低丙种球蛋白血症原因。
Objective: To investigate the effects of ketogenic diet on follicular helper T cells and B cells in children with refractory epilepsy. Methods: Thirty-two children with intractable epilepsy were enrolled in this study. Twenty normal control subjects were selected as control group. Flow cytometry was used to detect the proportion of Tfh cells and B cells before and after ketogenic diet treatment. The concentrations of IL-21 in serum were detected by ELISA. -PCR detection Blimp-1, Bcl-6, m TOR gene expression. Results: The number of Tfh cells decreased significantly after ketogenic treatment, the expression of Bf-1, the inhibitor of Tfh cell decreased, the expression of Bcl-6 decreased, and the levels of IL-21 and m RNA in serum decreased significantly The cells, CD27 + Ig D-BCD38bigb, showed a significant decrease compared to that after treatment. After treatment Tfh cells and various stages of B cells and m TOR expression there is a positive correlation. CONCLUSIONS: KD treatment reduced the ratio of Tfh cells to B cells or inhibited the function of Tfh cells and B cells via m TOR, which may be responsible for the presence of hypogammaglobulinemia in a minority of children undergoing KD therapy.
