正常甲状腺每日分泌甲状腺素(T_4)10nmol(其分泌率为三碘甲状腺氨酸,即 T_3的10~20倍);其中80%在代谢过程中被甲状腺以外的组织脱碘而转为 T_3(35%)及逆 T_3(r-T_3,45%),后者无激素作用。甲状腺直接分泌的 r-T_3每日仅3nmol。当患有全身性重病、大手术、饥饿及使用皮质激素时,T_4转为 r-T_3的部份增加:血清 r-T_3值增加而 T_3值减少,但 T_4值则持平或仅稍降。本文对巴西一教学医院收治的7位男性急症病员(6人系外伤,1人系十二指肠溃疡大出血,平均年龄38.9岁)作了研究。手术后24~36小时抽取桡动脉及殷静脉血作 PO_2、PCO_2、pH、Hb 的氧饱和度、HCO_3~-、血红蛋白(Hb)及血细胞压积测定;以放射免疫法测定静脉血内T_4、T_3及 r-T_3值。血氧含量依下列公式计算:
Normal thyroid secretes thyroxine (T_4) 10 nmol daily (the secretion rate is triiodothyronine, which is 10 to 20 times of T_3); 80% of them are deiodinated by tissues other than thyroid to T_3 35%) and inverse T_3 (r-T_3,45%), the latter hormone-free. Thyroid direct secretion of r-T_3 daily only 3nmol. When suffering from generalized serious illness, major surgery, hunger and the use of corticosteroids, T_4 to r-T_3 part of the increase: serum r-T_3 value increased T_3 value decreased, but the T_4 value was flat or only slightly lower. In this paper, 7 male emergency patients (6 trauma, 1 bleeding duodenal ulcer, mean age 38.9 years) admitted to a teaching hospital in Brazil were studied. Oxygen saturation, HCO_3 ~, hemoglobin (Hb) and hematocrit of PO_2, PCO_2, pH and Hb were measured by radial artery and venous blood from 24 to 36 hours after operation. Radioimmunoassay was used to determine the levels of T_4, T_3 and r-T_3 values. Blood oxygen content according to the following formula:
