目的:评价术前静脉注射氟比洛芬酯(flurbiprofen,FA)在人工流产手术麻醉超前镇痛中的应用价值。方法:将拟行人工流产手术的60例患者随机均分为两组。对照组(n=30)直接入手术室,静脉注射阿托品0.25mg、芬太尼0.05mg、丙泊酚2mg.kg-1,然后间断静脉注射丙泊酚0.5mg.kg-1维持麻醉;超前镇痛组(n=30)于麻醉准备室静脉注射阿托品0.25mg、芬太尼0.05mg、氟比洛芬酯0.2mg.kg-1,1min后入手术室静脉注射丙泊酚2mg.kg-1,然后间断静脉注射丙泊酚0.5mg.kg-1维持麻醉。记录两组患者围手术期平均动脉压(MAP)、心率(HR)、呼吸频率(RR)和脉搏氧饱和度(SpO2)。结果:两组患者围手术期MAP比较差异无统计学意义(P>0.05),而对照组HR在手术开始1、3、5min及清醒即刻明显高于超前镇痛组(P<0.05)。术后对照组中、重度疼痛的患者数明显高于超前镇痛组(P<0.05)。结论:FA超前镇痛可增强阿片类镇痛药的镇痛效果,抑制术后疼痛,维持稳定血流动力学指标,是较为理想的麻醉方案。
Objective: To evaluate the value of preoperative intravenous flurbiprofen (FA) in the treatment of advanced analgesia induced by induced abortion. Methods: Sixty patients scheduled for abortion were randomly divided into two groups. Control group (n = 30) directly into the operating room, intravenous injection of atropine 0.25mg, fentanyl 0.05mg, propofol 2mg.kg-1, and then intermittent intravenous infusion of propofol 0.5mg.kg-1 to maintain anesthesia; advanced Analgesia group (n = 30) intravenous injection of atropine 0.25mg, fentanyl 0.05mg, flurbiprofen axetil 0.2mg.kg-1,1min in the anesthesia preparation room into the operating room after intravenous injection of propofol 2mg.kg- 1, and then intermittent intravenous injection of propofol 0.5mg.kg-1 to maintain anesthesia. Perioperative mean arterial pressure (MAP), heart rate (HR), respiratory rate (RR) and pulse oxygen saturation (SpO2) were recorded. Results: There was no significant difference in perioperative MAP between the two groups (P> 0.05). HR in the control group was significantly higher than that in the preemptive analgesia group at 1, 3, 5min and immediately after awake (P <0.05). The number of patients with severe pain in the postoperative control group was significantly higher than that in the preemptive analgesia group (P <0.05). Conclusion: FA analgesia can enhance the analgesic effect of opioid analgesics, inhibit postoperative pain and maintain stable hemodynamic parameters, which is an ideal anesthetic solution.
