目的:通过临床药师参与解决肿瘤化疗患者的用药相关问题的实践,保障患者合理用药。方法:针对典型案例,临床药师从化疗药物的使用、不良反应的防治、患者用药教育与用药咨询等方面着手,协助医师解决患者用药过程中出现的问题。结果:临床药师在药学查房中及时发现并解决了患者用药的相关问题,提高了患者用药的疗效和安全性。结论:提供合理用药的个体化建议是药学服务的核心内容之一。
Objective: Through clinical pharmacists involved in the treatment of cancer chemotherapy patients with the practice of drug-related issues, to ensure that patients rational drug use. Methods: For typical cases, clinical pharmacists from the use of chemotherapy drugs, prevention and treatment of adverse reactions, the patient drug education and medication advice and so on, to help physicians to solve the problems arising in the course of medication. Results: The clinical pharmacist timely found and solved the problems related to the patient’s medication in the pharmacy rounds and improved the curative effect and safety of the patients. Conclusion: Individualized advice to provide rational use of medication is one of the core components of pharmacy services.
