[目的]探讨血清肿瘤标志物糖链抗原72-4(CA72-4)、糖链抗原19-9(CA19-9)、胚胎抗原(CEA)联合检测对胃癌的诊断价值。[方法]胃癌组为78例胃癌患者,良性病变组为78例胃部良性病变患者,健康对照组为78例医院体检的健康志愿者,检测各组患者血清CA72-4、CA19-9、CEA水平,对比各组CA72-4、CA19-9、CEA阳性率差异,分析单项及联合检测对胃癌的诊断价值。[结果]胃癌组血清CA72-4、CA19-9、CEA水平均显著高于良性病变组和健康对照组,差异均有统计学意义(P<0.05);胃癌组血清CA72-4、CA19-9、CEA阳性率分别为30.77%、25.64%、52.56%,均显著高于良性病变组和健康对照组,差异均有统计学意义(P<0.05);血清CA72-4、CA19-9、CEA单项诊断胃癌的敏感度均较低,特异度较高,三项联合诊断胃癌的特异度稍有降低,但敏感度显著提高。[结论]血清CA72-4、CA19-9、CEA联合检测可以提高诊断胃癌的敏感度,在胃癌的辅助诊断中具有重要的作用。
[Objective] To investigate the diagnostic value of combined detection of serum tumor marker carbohydrate antigen 72-4 (CA72-4), carbohydrate antigen 19-9 (CA19-9) and embryonal antigen (CEA) in gastric cancer. [Method] The gastric cancer group was 78 cases of gastric cancer, the benign group was 78 cases of benign gastric lesion, and the healthy control group was 78 healthy volunteers in the hospital. The serum levels of CA72-4, CA19-9, CEA Level, and compared the positive rates of CA72-4, CA19-9 and CEA in each group, and analyzed the diagnostic value of single and combined detection of gastric cancer. [Result] The levels of serum CA72-4, CA19-9 and CEA in gastric cancer group were significantly higher than those in benign disease group and healthy control group (P <0.05). The serum levels of CA72-4, CA19-9 , CEA positive rates were 30.77%, 25.64% and 52.56%, respectively, which were significantly higher than those in benign lesions and healthy controls (P <0.05). Serum CA72-4, CA19-9, The sensitivity of the diagnosis of gastric cancer were lower and higher specificity, the three combined diagnosis of gastric cancer slightly decreased the specificity, but the sensitivity was significantly improved. [Conclusion] The combined detection of serum CA72-4, CA19-9 and CEA can improve the sensitivity of gastric cancer diagnosis and plays an important role in the auxiliary diagnosis of gastric cancer.
