目的探讨参芎葡萄糖注射液和血液滤过联合治疗高脂血症性胰腺炎的临床效果。方法选取2011年7月至2015年4月于我院诊治的高脂血症性胰腺炎患者共61例进行研究,按照随机原则分为研究组(31例)和对照组(30例),对照组接受常规内科治疗和血液滤过治疗,研究组加用参芎葡萄糖注射液治疗。比较两组治疗前后甘油三酯(TG)、炎症因子、血浆黏度、全血黏度和APACHEⅡ评分,统计腹痛消失时间、住院时间、器官衰竭发生率和死亡率,并评定临床疗效。结果研究组治疗后的血清TG水平、炎症因子水平(IL-6、IL-8)、血浆黏度、全血黏度(高切、低切)和APACHEⅡ评分均较治疗前降低(P<0.05),且低于对照组(P<0.05);研究组与对照组总有效率分别为77.42%和53.33%,两组差异具有统计学意义(P<0.05);研究组患者腹痛消失时间和住院时间均短于对照组(P<0.05),器官衰竭发生率低于对照组(P<0.05);但两组死亡率对比差异无统计学意义(P>0.05)。结论参芎葡萄糖注射液和血液滤过联合治疗高脂血症性胰腺炎可迅速降低血脂水平,清除炎性介质,缩短治疗时间,改善预后,效果显著,值得推广。
Objective To investigate the clinical effects of Shenxiong glucose injection and hemofiltration in the treatment of hyperlipidemic pancreatitis. Methods A total of 61 patients with hyperlipidemic pancreatitis diagnosed and treated in our hospital from July 2011 to April 2015 were divided into study group (31 cases) and control group (30 cases) according to the random principle. Group received routine medical treatment and hemofiltration treatment, the study group plus Shenxiong glucose injection treatment. The triglyceride (TG), inflammatory factors, plasma viscosity, whole blood viscosity and APACHEⅡscores before and after treatment were compared between the two groups. The disappearance time of abdominal pain, hospitalization time, the incidence of organ failure and mortality were calculated. The clinical efficacy was also evaluated. Results After treatment, serum triglyceride level, inflammatory cytokines (IL-6, IL-8), plasma viscosity, whole blood viscosity (high and low cut) and APACHEⅡ scores in study group were lower than those before treatment (P < (P <0.05). The total effective rates of the study group and the control group were 77.42% and 53.33%, respectively, with significant difference between the two groups (P <0.05). The disappearance time and hospital stay of the study group (P <0.05), the incidence of organ failure was lower than that of the control group (P <0.05), but there was no significant difference in mortality between the two groups (P> 0.05). Conclusion Shenxiong glucose injection combined with hemofiltration in the treatment of hyperlipidemic pancreatitis can rapidly reduce blood lipid levels, clear inflammatory mediators, shorten the treatment time, improve the prognosis, the effect is significant and worth promoting.
