目的探讨机械瓣置换术后妊娠妇女妊娠期间全程服用华法林抗凝治疗对其生育儿童体格及智力发育的影响。方法将12名机械瓣置换术后的妊娠妇女所生育的儿童作为研究对象(试验组),分别进行体格及智力测定。测定指标包括:①体格发育指标:身高、体重、坐高、头围、胸围;②智力发育指标:智商和发育商。发育商适用于评估0～3岁(0～36月)年龄阶段儿童的智力发育情况。发育商指标包括:适应性、大运动、精细运动、语言、个人-社会。智商适用于评估4岁及4岁以上儿童的智力发育情况。按照试验组与对照组1∶2配对的方法选取正常妇女生育的24名儿童作为对照组,比较两组各项指标之间的差异。结果机械瓣置换术后妊娠妇女生育的儿童与正常妇女生育的儿童在0～3岁、4～6岁和6～12岁三个年龄阶段的体格发育指标及智力发育指标的差异均无统计学意义(P>0.05)。结论机械瓣置换术后的妊娠妇女妊娠期全程口服华法林抗凝治疗在每日服药剂量≤5 mg的情况下,未观察到其对儿童的体格及智力发育有负面影响。
Objective To investigate the effect of warfarin anticoagulation therapy on the physical and mental development of pregnant children during pregnancy after mechanical valve replacement. Methods Twelve children born to pregnant women who had undergone mechanical valve replacement were selected as study subjects (experimental group), and their physical and mental parameters were measured respectively. Measurement indicators include: physical development indicators: height, weight, sitting height, head circumference, chest; ② mental development indicators: IQ and development. Developers are suitable for assessing the mental development of children aged 0-3 years (0-36 months). Developmental indicators include: adaptability, big exercise, elaborate exercise, language, personal-social. IQ applies to assess the mental development of children aged 4 years and over. According to the method of pairing test group and control group 1: 2, 24 children born in normal women were selected as the control group, and the differences between the two groups were compared. Results There was no significant difference in the physical and mental developmental indices of children aged 3 ~ 4, 6 ~ 6 and 6 ~ 12 years old after childbirth in both pregnant women and normal women after mechanical valve replacement Significance (P> 0.05). CONCLUSIONS: Warfarin anticoagulant therapy in pregnant women after mechanical valve replacement during pregnancy is not observed to have a negative impact on children’s physical and mental development at daily doses of ≤5 mg.
