目的了解支气管肺发育不良(BPD)早产儿肺功能的特点,为疾病的诊断、治疗提供依据。方法研究对象为60例体重<1.7 kg,胎龄<34周的早产儿,其中BPD组和对照组各30例,在体重>1.8 kg以上、出院时进行潮气呼吸肺功能测定,比较两组差别。结果BPD组在达峰容积比VPEF/VE(23.96±6.13)%和达峰时间比TPEF/TE(21.03±7.20)%较对照组明显减低(P<0.05),潮气呼吸呼气峰流速(PTEF)BPD组较对照组稍高(P<0.05)。其他肺功能指标均与对照组差异无显著性。结论支气管肺发育不良早产儿的潮气肺功能变化提示阻塞性通气障碍,与BPD的严重程度有关。对于BPD的早产儿出院后动态肺功能测定有助于早期发现后期患呼吸系统疾病的风险。
Objective To understand the characteristics of pulmonary function in premature infants with bronchopulmonary dysplasia (BPD) and provide evidence for the diagnosis and treatment of the disease. METHODS: Sixty preterm infants with body weight <1.7 kg and gestational age <34 weeks were enrolled in this study. Among them, 30 cases in BPD group and 30 cases in control group each received more than 1.8 kg of body weight. During discharge, the pulmonary function test was performed. . Results Compared with the control group, the peak volume of the BPD group was significantly lower than that of the control group (P <0.05) at peak volume ratio of VPEF / VE (23.96 ± 6.13)% and peak time of TPEF / TE ) BPD group slightly higher than the control group (P <0.05). Other lung function indicators were no significant difference with the control group. Conclusion The changes of tidal pulmonary function in premature infants with bronchopulmonary dysplasia suggest obstructive ventilatory disturbance, which is related to the severity of BPD. Post-discharge dynamic pulmonary function testing in preterm infants with BPD is at risk of early detection of late-stage respiratory disease.
