作者认为,CML发生急变的趋势可能与体内白血病干细胞的总量有关.用标准剂量的细胞毒药物及放疗对减少体内白血病干细胞影响很少,但用大剂量化疗药物后再以较少量的自体造血干细胞解救的治疗方法则可显著减少体内白血病干细胞的总量,从而能延迟急变的发生及延长生命.如果从外周血分离出的干细胞自我复制能力有限,则这种治疗将具有特殊的价值.作者报道了14例Ph阳性的CML慢性期病人用外周血干细胞进行自体移植的结果.其中11例是用马利兰(16mg/kg)和马尔法兰(60mg/m~2)预处理后进行的,另3例是在HLA匹配的同胞骨髓细胞移植
The authors believe that the trend of rapid changes in CML may be related to the total amount of leukemia stem cells in the body.
