目的探讨差异表达基因谱预测下咽鳞癌化疗敏感性的临床应用。方法回顾性分析41例接受诱导化疗的下咽鳞癌临床病理资料,自不同化疗敏感性的下咽鳞癌差异基因表达谱中,随机选取CDDN1、FAS、STAT基因作为差异基因表达谱标志,检测各基因表达,Logistic回归模型分析差异表达基因谱和单一基因预测化疗敏感性的准确性。结果在41例下咽鳞癌中,化疗敏感19例,其中CDDN1、FAS、STAT3和差异基因表达谱阳性表达率分别为57.9%(11/19)、57.9%(11/19)、47.4%(9/19)和78.9%(15/19),均高于化疗不敏感组,差异有统计学意义(P<0.05)。CDDN1、FAS、STAT3和差异基因表达谱阳性表达与化疗敏感符合率分别为57.9%(11/19)、63.2%(12/19)、47.3%(9/19)和78.9%(15/19)。FAS和差异基因表达谱预测下咽鳞癌化疗敏感性的准确率分别为68.3%(P<0.05)和85.4%(P<0.001)。结论差异基因表达谱可较准确地预测下咽鳞癌化疗敏感性,准确率高于单一基因。
Objective To investigate the clinical application of differentially expressed genes in predicting the chemosensitivity of pharyngeal squamous cell carcinoma. Methods The clinical data of 41 squamous cell carcinoma of the hypopharyngeal squamous cell carcinoma patients undergoing induction chemotherapy were retrospectively analyzed. CDDN1, FAS and STAT genes were randomly selected from the differential gene expression profiles of squamous cell carcinoma of different chemosensitivity and detected as differential gene expression markers Each gene expression, Logistic regression model analysis of differentially expressed gene profiles and single gene prediction of the accuracy of the chemosensitivity. Results The positive rate of CDDN1, FAS, STAT3 and differential gene expression in 41 cases of squamous cell carcinoma of the hypopharyngeal squamous cell carcinoma was 57.9% (11/19), 57.9% (11/19) and 47.4% 9/19) and 78.9% (15/19), respectively, which were significantly higher than those in chemotherapy-insensitive group (P <0.05). The coincidence rates of CDDN1, FAS, STAT3 and differential gene expression were 57.9% (11/19), 63.2% (12/19), 47.3% (9/19) and 78.9% (15/19) . The accuracy of FAS and differential gene expression profiling were 68.3% (P <0.05) and 85.4% (P <0.001) respectively. Conclusion The differential gene expression profile can predict the chemosensitivity of hypopharyngeal squamous cell carcinoma more accurately than the single gene.
