目的寻找先天性马蹄内翻足(congenital talipes equinovarus,CTEV)前足内旋的客观评价指标,测定并分析5个跖跟角(跖骨相对跟骨的内偏角度)的差异性,为临床评价提供依据。方法自2010以来在我院诊断为先天性马蹄内翻足的患儿36例,其中男18例,女18例;年龄5d~71个月,平均13.5个月。在双足X线正位片上分别测量患足和健足的5个跖跟角,首先比较同一患儿的患足与健足之间跖跟角的差异显著性,再对比五组跖跟角的患健侧差值之间的差异程度。结果 CTEV的5个跖跟角均大于健康足,且差异具有统计学意义(P<0.05);5组跖跟角的患健侧差值之间无明显差别(P>0.05)。结论第1~5跖跟角可作为先天性马蹄内翻足前足内旋程度的评价指标。
Objective To find the objective evaluation index of pronation of congenital talipes equinovarus (CTEV), and to determine and analyze the difference of five heel angles (the angle between the metatarsal and the calcaneus) to provide the basis for clinical evaluation . Methods Thirty-six children with congenital clubfoot diagnosed in our hospital since 2010, including 18 males and 18 females, aged from 5 days to 71 months with an average of 13.5 months. In the biped X-ray anteroposterior film were measured foot and foot health five metacarpal heel angle, the first comparison of the same child’s foot and foot health between the significant difference between the plantar and angle, and then compared the five groups of plantar with angle The degree of difference between the affected side difference. Results The 5 plantar heel angles of CTEV were all higher than that of healthy foot, and the difference was statistically significant (P <0.05). There was no significant difference between healthy group and control group (P> 0.05). Conclusion The first to fifth metacarpal heel angle can be used as an index for evaluating the degree of pronation of congenital clubfoot.
