目的:探讨慢性阻塞性肺疾病(COPD)病情严重程度与血瘀证的关系。方法:80例COPD急性加重期患者,根据肺功能分为3组。肺功能2级(组1)22例,肺功能3级(组2)30例,肺功能4级(组3)28例。根据患者的临床表现、体征及血气分析结果评估患者的病情情况,并进行血瘀证证候评分,血小板参数及凝血功能的测定。结果:组1患者22例,合并呼吸衰竭2例;组2患者30例,合并呼吸衰竭8例;组3患者28例,合并呼吸衰竭10例;3组差异有显著性意义(P<0.05)。证候积分组1(34.00±6.55)分,组2(42.42±6.98)分,组3(50.45±4.79)分。3组比较,差异有显著性意义(P<0.05)。血瘀证症候积分越高,血瘀证的程度越明显。3组血小板(BPC)、平均血小板体积(MPV)、血小板分布宽度(PDW)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)各自比较,差异均有显著性意义(P<0.05);组1与组3比较,组3的BPC、APTT明显低于组1,而MPV、PDW、FIB明显高于组1,差异均有非常显著性意义(P<0.01)。结论:CCPD急性加重期病情严重程度与血瘀证有相关性。
Objective: To investigate the relationship between the severity of chronic obstructive pulmonary disease (COPD) and blood stasis syndrome. Methods: Eighty patients with acute exacerbation of COPD were divided into 3 groups according to pulmonary function. Twenty-two patients had pulmonary function grade 2 (group 1), 30 patients had grade 3 pulmonary function (group 2), and 28 patients had grade 4 pulmonary function (group 3). According to the patient’s clinical manifestations, signs and blood gas analysis results to assess the patient’s condition, and blood stasis syndrome syndrome score, platelet parameters and coagulation function determination. Results: There were 22 patients in group 1 and 2 patients with respiratory failure, 30 patients in group 2 and 8 patients with respiratory failure, 28 patients in group 3 and 10 patients with respiratory failure. There were significant differences among the 3 groups (P <0.05) . The score of syndrome group was 1 (34.00 ± 6.55) points, group 2 (42.42 ± 6.98) points and group 3 (50.45 ± 4.79) points. There was significant difference between the three groups (P <0.05). The higher the blood stasis syndrome score, the more obvious the degree of blood stasis syndrome. There were significant differences between the three groups in terms of BPC, MPV, PDW, APTT and fibrinogen (P <0.05) ). Compared with group 3, BPC and APTT of group 3 were significantly lower than group 1, and MPV, PDW and FIB were significantly higher than group 1 (P <0.01). Conclusion: The severity of CCPD during acute exacerbation of CCPD is correlated with blood stasis syndrome.
