目的比较甲状腺腺叶次全切除术与甲状腺腺叶切除术治疗甲状腺单发肿瘤的临床效果。方法选择2012年7月至2014年6月间收治的甲状腺单发肿瘤患者78例,采用随机数字表法分为对照组和治疗组,每组39例。对照组患者应用甲状腺腺叶次全切除术,治疗组患者采用甲状腺腺叶切除术,观察两组患者术中出血量、手术时间、引流时间和术后引流量以及术后并发症。随访6个月,观察术后甲状腺激素恢复时间。结果治疗组患者术中出血量和手术时间分别为(82.41±7.93)ml和(119.86±13.50)min,均低于对照组,差异有统计学意义(P<0.05)。观察组患者术后并发症发生率为10.3%(4/39),低于对照组的25.6%(10/39),差异有统计学意义(P<0.05)。观察组患者术后甲状腺激素恢复时间为(142.71±11.84)d,低于对照组的(169.53±12.65)d,差异有统计学意义(P<0.05)。结论甲状腺腺叶切除术治疗甲状腺单发肿瘤术中出血量少,手术时间短,且术后并发症显著降低,术后甲状腺激素恢复较快,安全有效,值得临床推广应用。
Objective To compare the clinical effects of subtotal thyroidectomy and thyroidectomy for the treatment of single thyroid tumors. Methods Seventy-eight patients with thyroid solitary tumors who were treated between July 2012 and June 2014 were randomly divided into control group and treatment group with 39 cases in each group. Patients in the control group were treated with subtotal thyroidectomy. Patients in the treatment group were given thyroidectomy for thyroidectomy. Blood loss, operation time, drainage time, postoperative drainage volume and postoperative complications were observed in two groups. The patients were followed up for 6 months to observe the postoperative thyroid hormone recovery time. Results The intraoperative blood loss and operative time in the treatment group were (82.41 ± 7.93) ml and (119.86 ± 13.50) min, respectively, which were significantly lower than those in the control group (P <0.05). The incidence of postoperative complications in the observation group was 10.3% (4/39), which was lower than that in the control group (25.6%, 10/39) (P <0.05). The recovery time of thyroid hormone in the observation group was (142.71 ± 11.84) d, which was lower than that in the control group (169.53 ± 12.65) d, the difference was statistically significant (P <0.05). Conclusions Thyroidectomy for the treatment of thyroid tumors alone has less bleeding and shorter operative time, and the postoperative complications are significantly reduced. The postoperative thyroid hormone recovery is quick, safe and effective, which is worthy of clinical application.
