目的:比较平台转移设计种植体与传统对接连接种植体在功能负重与非功能负重状态下种植体颈部骨组织的动态变化,细化平台转移设计对种植体颈部骨组织的影响因素。方法:将6只犬随机编为1~6号,拔除双侧下颌第二、三、四前磨牙,3个月后按设计顺序植入Ankylos、GS-Ⅱ、US-Ⅱ种植体共36颗。术后3个月时行二期手术,4个月时右侧取模,4.5个月时戴冠,12个月时处死动物。植入即刻、第3、6、9、12个月时摄片。测量肩台—骨结合冠方最高点(IS-CLB)、肩台—颈部牙槽嵴顶(IS-BC)距离。根据所用种植体的不同分为A、G、U 3组,采用SPSS11.5软件包对数据进行统计学分析。结果:A、G组负重侧与非负重侧的IS-CLB、IS-BC在种植术后第6、9、12个月时与U组比较,均有显著性差异,A、G组间无差异。各组自身负重侧与非负重侧比较,均无差异。各组自身比较,种植即刻与3个月时无显著差异,3个月与6个月、6个月与9个月、9个月与12个月时比较,均有显著性差异。结论:潜入式种植体颈部骨组织的显著性变化始于种植二期术后。种植体植入并行使功能12个月,颈部骨组织的吸收与功能负重无关。
OBJECTIVE: To compare the dynamic changes of bone tissue in the neck of implants under the functional load-bearing and non-functional load-bearing implants and to compare the influencing factors of the platform transfer design on the bone structure of implant neck. Methods: Six dogs were randomly assigned to No. 1-6. The second, third and fourth premolars of both mandibular lower jaws were removed. Three months later, 36 Ankylos, GS-Ⅱ and US-Ⅱ implants were implanted in order of design . The second operation was performed at 3 months after operation, the right side was taken at 4 months, the crowned at 4.5 months, and the animals were sacrificed at 12 months. Immediately implanted, 3,6,9,12 months when the film. The distance between the highest point of the shoulder-bone union crown (IS-CLB) and the shoulder-neck alveolar ridge (IS-BC) was measured. According to the different implants used were divided into A, G, U 3 groups, using SPSS11.5 software package for statistical analysis of the data. Results: Compared with U group, the IS-CLB and IS-BC of both load-bearing and non-load-bearing sides of group A and G were significantly different at 6, 9 and 12 months after implantation difference. Each group own weight side and non-weight side, no difference. There was no significant difference between each group immediately after planting and 3 months, 3 months and 6 months, 6 months and 9 months, 9 months and 12 months. CONCLUSIONS: Significant changes in neck bone structure of submerged implants started after the second stage of implantation. Implants implanted and exercise function for 12 months, neck bone tissue absorption and functional load has nothing to do.
